Site work for a bridge / tunnel in Colo, Iowa. Completed plans call for site work for a bridge / tunnel.

Project is for replacement of five culvert locations from 0.5 mi. north of US 30 to IA 175 in Hubbard on US 65. Project will include a detour route, HMA full depth paving, removal of existing culverts or pipes, installation of new culverts and pipes, contractor furnished embankment in place, topsoil stripping and placement, paved HMA shoulders, granular shoulders, granular entrance. The scope of work for this project covers the supplying of all labor, tools, materials, equipment, services and appurtenances to accomplish the work specified and indicated on the contract drawing and to provide for proper disposal of all waste and demolition debris in accordance with federal, state, and local regulations. The scope of this specification includes the packaging and removal of these materials from the identified structures, offsite transport, and disposal/recycling in accordance with federal, state and local regulations described in this document. The work shall be performed to the complete satisfaction of the Department in accordance with the current EPA and OSHA regulations, State Labor and Industry and Department of Environmental Protection regulations and any other applicable State and Local Government regulations. A. The scope of work for this project covers the supplying of all labor, tools, materials, equipment, services and appurtenances to accomplish the work specified and indicated on the contract drawing. The work shall be performed to the complete satisfaction of the Department in accordance with the current EPA and OSHA regulations, State Labor and Industry and Department of Environmental Protection regulations and any other applicable State and Local Government regulations. B. Contractor shall submit an Action Plan which describes specifically how abatement work is to be completed for each abatement phase and how material segregation will be conducted. At a minimum the Action Plan shall address work area preparation, work practices, temporary heat, decontamination location and procedures, estimated completion dates, respiratory protection, worker protection, and disposal. Approval of the Action Plan must be obtained through the Professional prior to the start of work. Procedures outlined in the Action Plan must be followed throughout the abatement phase. Any changes in the Action Plan require prior approval from the Professional. C. Surveys conducted by the Department indicate abatement activities will be needed in the following areas. This specification may not encompass all asbestos materials present on the site. The Contractor is responsible for identification and abatement of any and all asbestos materials encountered. See drawings for locations and quantities. These should not be considered to be inclusive of all asbestos materials potentially present at the site. Demolition may be required to access the ACM. Proposals received after the bid deadline will not be read. Proposals containing any reservations not provided for in the forms furnished will be rejected. The Iowa DOT reserves the right to waive technicalities and reject any or all bids. Unless otherwise specified, a bidder must submit a proposal guaranty/bid bond with each bid as per Article 1102.11 of the Standard Specifications. Failure to execute a contract and file an acceptable performance bond and certificate of insurance within 30 days of the approval date for awarding the contract will be just and sufficient cause for the denial of the award and forfeiture of the proposal guaranty. The Iowa DOT (or other approved contracting authority) hereby notifies all bidders that it will affirmatively ensure that in any contract entered into pursuant to this advertisement, disadvantaged business enterprises will be afforded full opportunity to submit bids in response to this invitation and will not be discriminated against on the grounds of race, color, or national origin in consideration for an award. Minimum wage rates for all federal-aid projects have been predetermined by the U.S. Secretary of Labor and are set forth in the specifications. However, this does not apply to projects off the federal-aid highway system. All federal-aid projects are subject to the Contract Work Hours Standards Act, as amended (title I of Pub.
