Site work for a water / sewer project in Bethany, Pennsylvania. Completed plans call for site work for a water / sewer project.
Sealed proposals will be received by Honesdale Borough (OWNER) for the Terrace Street Stormwater Improvements project within Honesdale Borough, Wayne County, Pennsylvania. This Project is considered to be construction as defined by the Pennsylvania Prevailing Wage Act 442 of 1961, and is subject to prevailing wage. Determination is attached. There is an Erosion and Sediment (E&S) Control Plan that has been approved for the project. If the Contractor proposes changes to the approved plan, they shall be submitted to the Borough for approval. It will be the Contractors responsibility to obtain approval from the reviewing agency for the proposed changes, if acceptable to the Borough. Easements from the individual property owners have been obtained by the Honesdale Borough. It will be the responsibility of the Contractor to review all of the Easements and restrict all operations to within the Easement limits. Honesdale Borough, Wayne County, PA is exempt from all excise taxes and sales tax. No taxes shall be charged to the Borough. The successful bidder shall be retained by the Owner to provide construction services through the duration of the project. The work is required to be completed prior to July 29, 2023 within a timely manner. Contractor shall pay Owner $500 for each day that expires after July 29, 2023 that the work is not complete. The Owner reserves the right to reject any or all proposals and to select the proposal that it judges to be in the best interest of the Owner. The contract is subject to the approval of the Owner and is effective only upon their approval. All Contractors are bound by the deadline and submission requirements of this RFP. All proposals shall remain effective subject to Owner review and approval for a period of thirty (30) days from the deadline for submitting proposals. The Owner may initiate negotiations with the Contractors submitting the proposal or seek additional proposals on an informal or formal basis during the thirty (30) day period that proposals must remain effective. All inquiries, questions, etc. once construction has started shall be forwarded to the Project Manager for the Honesdale Borough, Dan Brown. Email dpwhdle@ptd.com or 570-470-3099. Direct contact with any Owner employee without the expressed permission of the Project Manager on the subject of this proposal, is strictly forbidden. Violation of this paragraph may result in disqualification of your proposal. The Owner, or its agencies, is not liable in any way for any costs incurred by the Contractor's in the preparation of their proposals in response to the RFP, nor for the presentation of their proposals and/or participation in any discussion or negotiations. The Owner, or its agencies, reserves the right to accept in part or in whole any or all proposals submitted or to waive any technicality or minor irregularity in a proposal. Unreasonable failure of a Contractor to promptly supply the Owner with information with respect to responsibility may be grounds for a determination of non-responsibility. All Proposals, RFPs, are contingent upon Owner appropriations budgetary constraints. The RFP describes the requirements and response format in sufficient detail to secure comparable proposals, recognizing that various proponent approaches may vary widely. Any proposal that differs from the described format may be considered non-responsive and rejected. Any and all exceptions to the RFP must be listed on an item-by-item basis and cross-referenced with the RFP document. If there are no exceptions, Contractor must expressly state that no exceptions are taken. All proposals shall be valid for thirty (30) days from the date of the RFP opening and become the property of the Owner. If negotiations result in modifications to the RFP, then thirty (30) days will commence from the date of the receipt of the new proposal. This period may be extended by mutual written agreement between the Respondent and the Owner. All proposals will be evaluated based in part on the technical and professional expertise and experience of the firm, the proposed method and the procedures for completion of the work and the cost of the proposal. The apparent ability of the firm to be independent and objective in performing the requested work will also be considered. The Owner reserves the right to add additional evaluation criteria as it deems appropriate SECTION 3: PROPOSAL A. Proposal Requirements 1. Proposal shall include 10% Bid Bond 2. Contractor to provide 100% Performance and Payment Bond upon Notice of Award 3. Prices shall include Prevailing Wage Rates 4. Contractor shall be responsible for contacting PA-1-Call 5. Contractor to schedule any required inspections with Honesdale Borough or any other regulatory agency as needed. B. Items to be included in Proposal 1. Mobilization single mobilization 2. Traffic Control as required during mobilization to maintain one traffic lane open at all times 3. Tree and vegetation removal only as needed to safely perform work 4. Erosion and Sediment Controls (Rock Construction Entrance, Cofferdam, Bypass Flume, Filter bag, Erosion Control Blanket, Outlet Protection, etc.) 5. Removal and disposal of existing components per plans 6. Supply and installation of proposed components per plans 7. Permanent stabilization and restoration per plans 8. Protection of existing trees and structures on the affected properties to remain (any accidental or inadvertent damage to private or public property during the project will be restored at the contractor s expense. Brief Project Description The project will consist of replacing approximately 270 linear feet of existing stormwater conveyances (stone headwalls, 36" CMP, catch basin inlets, open channels, 30" CMP) with a new system of 465 linear feet of 36" HDPE stormwater pipe, headwalls, catch basin inlets and outlets. A 20 ft section of existing 36" CMP will be capped and grouted to remain in place. The project includes restoration of the site including, but not limited to fences, walls and driveways on the affected private properties. 1. Project Details: The project will consist of: o Installation of all new Storm Inlet Box and Frames with associated new HDPE Stormwater Pipe connections. o All pipe penetrations to concrete structures shall be completely sealed with cement grout. o Concrete and pavement restoration on all affected properties and roads as required. Sawcuts are required for all concrete or asphalt replacement. o Vegetation restoration to include 4" minimum topsoil, seed and mulch. Area should be positively graded to account for future settling. B. Project Location The project is located between Terrace and Ridge Streets in Honesdale, PA between the 300 and 400 block of Terrace Street. Access to the property will be from either Terrace Street or Ridge Street at the locations shown on the plans.
