The City of Blackwell/Blackwell Facilities Authority reserves the right to reject any bid or all bids, to waive irregularities or informalities in any bid and to award the bid to the bidder who, in the opinion ofthe Blackwell City Commission/BFA Trustees, offers the proposal which appears to be ofthe best interest ofthe City ofBlackwell/Blackwell Facilities Authority A Business Relationship Affidavit, Non-Collusion Affidavit and a Cashier Check, a Certified Check or a Surety Bid Bond in the amount of five percent (5%) ofthe bid shall accompany the sealed proposal of each bidder. Deposits will be returned to the unsuccessful bidders and to the successful bidder upon approval of the Contract and submission of a 100% Performance Bond, 100% Statutory Bond and a 100% Maintenance Bond. Do not include state or local sales tax in your bid. The City of Blackwell will appoint the successful bidder its agent for the purpose of receiving tax exempt status on the purchase of material In addition, the bidder shall comply with all relevant and applicable provisions of the State of Oklahoma Competitive Bidding Act of 1974 To bid, all relevant documents and materials pertaining to the bid, including the bidding form containing the bidder's name, address, all contact numbers and e-mail addresses and bid amount, fully executed non-collusion affidavit, warranty, with all documents executed by the authorized representatives, should be placed in separate sealed envelopes and marked as
