Contractors shall use only the classifications and wage rates set forth in the United States Department of Labor (USDOL) wage decision found on payrolls submitted. Additionally, please note that the wage modification in effect at the time of the project sale date, shall be used by all contractors. Each bid must contain the full name of every person, persons, or company interested in same and be accompanied by a bid guaranty in the form of either a bond in an amount equal to one hundred percent (100%) of the total bid drawn in favor of the City of Fairview Park and the Ohio Department of Transportation (dual obliges), or a consent of surety together with a certified check or cashier's check or letter of credit on a solvent bank, made payable to the City of Fairview Park in an amount not less than ten percent (10%) of the total amount of the bid as prescribed in Ohio Revised Code Section 153.54. Only ODOT pre-qualified contractors are eligible to submit bids for the PROJECT. Pre-qualification status must be in force at the time of bid, at the time of award, and through the life of the construction contract. Disadvantaged Business Enterprise (DBE) Requirement. DBE participation goals (subcontracts, materials, supplies) have been set on this project for those certified DBEs pursuant to Title 23, U.S.C. section 140(c) and 49 CFR, Part 26, and where applicable qualified to bid with ODOT under Chapter 5525 of the ORC. The DBE Goal for this project is 8%. The contractors must also be in good standing in the Drug-Free Workplace program (DFWP) or a similar program approved by the Bureau of Workers Compensation, and requires the same of any of its subcontractors. Record Retention: As the LPA, ODOT or the United States Government may legitimately request from time to time, the Contractor agrees to make available for inspection and/or reproduction by the LPA, ODOT or the United States Government, all records, books, and documents of every kind and description that relate to this contract.
