Site work and paving for a mixed-use development in Millville, New Jersey. Completed plans call for site work for a road / highway; for paving for a road / highway; sidewalk / parking lot; and sidewalk / parking lot.

The work to be performed under this contract generally consists of removing existing roadway and sidewalk patches and performing final pavement restorations at various street locations throughout the City of Millville. All bids must be on the proposal form attached to the Specifications in the manner designated therein, must be enclosed in sealed envelopes bearing the name and address of the bidder and the name of the project clearly marked "PROPOSAL FOR 2023 STREET PATCHING AND UTILITY OPENING REPAIRS" and addressed to the Board of Commissioners of the City of Millville, 12 S. High Street, Millville, New Jersey, 08332. The bid must be accompanied by the following: Bidder's Checklist; Bid Proposal Form; Bid Guarantee in the form of a Bid Bond or Certified Check or Cashier's Check in the amount of 10% of the total amount of the bid proposal, not to exceed $20,000, made payable to the City of Millville; Consent of Surety; Acknowledgement of Receipt of Addenda or Revisions; Stockholder Disclosure Certification; Business registration Certificate issued by the N.J. Division of Revenue; NonCollusion Affidavit; Public Works Contractor registration; New Jersey Debarred List Affidavit; Indemnity and Hold Harmless Agreement; and Affirmative Action Compliance Notice. This bid is being solicited through a fair and open process in accordance with N.J.S.A. 19:44A-20.5. Bidders are required to comply with the requirements of N.J.S.A. 10:5-31 et seq. (P.L. 1975, c 127, as amended and supplemented) and N.J.A.C. 17:27 et seq., Affirmative Action requirements of the State of New Jersey. The Board of Commissioners reserves the right to reject any or all proposals in whole or in part and to waive such informalities as may be permitted by law. The work to be performed under this contract generally consists of removing existing roadway and sidewalk patches and performing final pavement restorations at various locations throughout the City of Millville. The City has identified three (3) case conditions for roadway repairs and four (4) case conditions for sidewalk repairs which shall be utilized in executing the work under this contract. Work will be performed within public rightof-ways City Streets, County Roads, and State Highways. Specific details of the work requirements are outlined in "Section 700 - Work to be Performed and Methods of Payment" as is contained herein. Mandatory response time for work requested is 24-48 hours. A response shall include communication and onsite assessment to determine the scope of work. The scope of work shall be completed within 10-15 Business days unless approved or directed otherwise by the Engineer.
