Written Comments Deadline May 3, 2023 by 2:00 pm MT The purpose of this Request for Proposal is for the selection of a General Contractor on behalf of EL PASO COUNTY HOSPITAL DISTRICT d/b/a UNIVERSITY MEDICAL CENTER OF EL PASO, (hereinafter UMC). The General Contractor will be responsible for the requisite material, labor, equipment and services necessary for the renovation of the existing Pharmacy in El Paso Children's Hospital. Reference Exhibit B for Scope of Work. The selection of the firm will be at the sole discretion of UMC about which Offeror's Proposal provides the best value to UMC based on the evaluation factors set forth in Section 4 herein. Project Completion Schedule: Offerors are advised that the work contemplated under this project is coordinated with other commitments of the Owner, and it is therefore imperative that the work be completed on schedule. Each Proposal must be accompanied by the Surety Company Bond, Cashier's Check, or a bank Certified Check, in the sum of not less than Five Percent (5%) of the maximum Proposal Amount, payable to University Medical Center of El Paso, with the understanding that the Offeror will not withdraw his Proposal for a period of thirty (30) day after the dates set for the Owner receipt of Proposals on the project. The Contractor shall furnish and pay for a good and approved Surety Bond for the full amount 100% of the Contract price as a Payment and Performance Bond. These bonds shall be with a company satisfactory to the Owner, and shall be accompanied by the Surety Power of Attorney. The Payment and Performance Bond shall be executed on a form approved by the Owner. The Contract shall not become binding and effective until these bonds and Powers of Attorney are approved by the Owner. Agreement Form: See Sample Agreement referred to in Exhibit A FIXED PRICE CONSTRUCTION AGREEMENT BETWEEN OWNER AND CONTRACTOR. The information included in the Exhibit will be finalized during the contract negotiation process. Sales Tax: All work included under this Contract is exempt from State Sales Tax, and the Contractors Proposal should not include sales tax. Building Permit: The cost of all applicable permits, fees, etc., shall be included in the Proposal. Prevailing Wage: The Offeror is aware that and hereby agrees that not less than the prevailing wage rates adopted by the El Paso County Hospital District Board of Managers on June 13, 2017 shall be paid to all workers on the Project. A copy of the prevailing wage rates and the Prevailing Wage Rate Compliance Procedures are attached as Exhibit C. . An Officer of the responding company must sign the Response Agreement AS IS, DO NOT change or retype the form. If for any reason, our specifications and/or Response Agreement are altered in the response, University Medical Center of El Paso may reject the proposal and the respective offeror may not be considered.
