STATEMENT OF WORK Remodel Building 5 for OI&T, Fort Wayne Project # 610A4-24-101 INTRODUCTION/BACKGROUND The VA Northern Indiana Health Care System, Fort Wayne Campus located at 2121 Lake Avenue, Fort Wayne IN 46805 has identified the need to remodel building 5 to improve handicap accessibility and correct structural and life safety deficiencies identify in the FY2018 Facilities Condition Assessment (FCA) performed by the VISN. JUSTIFICATION Building 5 deficiencies were noted discrepancies in the 2018 VISN 10 Facility Condition Assessment and the remodel is necessary to ensure employee life safety and for the facility to remain code compliant. SCOPE OF WORK The architectural and engineering (A/E) firm shall provide professional services necessary to develop 100% design for project 610A4-24-101, Remodel Building 5 for OI&T at the VA Northern Indiana Health Care System, Fort Wayne Campus located at 2121 Lake Avenue, Fort Wayne IN 46805. This project will remodel of 2,250 square feet in Building 5 on the Fort Wayne campus to improve handicap accessibility, correct structural and life safety deficiencies, new interior design and finishes as identify in the FY2018 Facilities Condition Assessment (FCA) performed by the VISN. Project design to remain within the constraints of the construction budget consistent with the Veterans Affairs (VA s) project team goals as prioritized during project meetings and field surveys. Conduct preliminary project scope meetings and site survey investigations to develop the necessary elements for conceptual layout schemes with proposed alternatives consistent with the VA s project team goals as prioritized during preliminary project meetings, field surveys and studies. The scope of this project shall consist of, but not limited to, surveying the proposed site, existing conditions, verifying record drawings, verifying and analyzing utility systems within the VA Northern Indiana Health Care System, Fort Wayne Campus in conjunction with preparation of schematic design and design development. The project design shall consist of a construction statement of work, demolition and construction drawings which will include asbestos abatement as necessary and an Infectious Control Risk Assessment (ICRA) plan and specification, renderings, as well as technical specifications and cost estimates for bid purposes per the A/E design submission instructions document. The drawings shall meet all applicable federal governing codes. Drawings shall be stamped by registered architects and/or professional engineer disciplines. Design shall comply with applicable sections of the Technical Information Library on the Office of Construction and Facilities Management (CFM) website www.cfm.va.gov. Project is to be designed to comply with the latest codes as outlined in the VHA Program Guide PG-18-3, Topic 1 Codes, Standards, and Executive Orders on the TIL as well as other applicable Topics under PG 18-3. A/E will be responsible for complete functional design and provide bidding documents meeting those requirements. Applicable chapters of VA Master Construction Specifications will be prepared by the A/E, including any necessary adaptations. To include but not limited to PG 18-9 Chapter 232 Office of Information and Technology (OIT) Space Planning Criteria; FA 5: Staff and Administrative Area shall be utilized as a basis for design planning. VA HVAC Design Manual, VA Sustainable Design and Energy Reduction Manual, Infrastructure Standard for Telecommunications Spaces - v3.1 and, ASHRAE Standards 90.1-2019 shall apply to specific design requirements. Refer to Contracting & Architect/Engineer Information and Technical Information Library on the CFM website www.cfm.va.gov for specific Contract Document requirements. The Architect/Engineer performing the work shall include items identified in attachment (1) of this document. Line items a. and b. shall be included as a bid alternate. Installation of 3 story elevator. Attachment (1) line items 12 and 49. Wood-clad double-glazed windows. Attachment (1) line item 42. A/E shall develop plan and deduct alternates for line items a. and b. for a project award not to exceed $1,821,600. 10% bid deductions and/or add alternatives must be built into the scope of the project to allow for flexibility in the construction award. Other considerations for design include Architecture, Structural, Plumbing, Mechanical, Electrical, and Technology. The A/E shall utilize the VA Design Manuals by discipline PG-18-10, and Space Planning Criteria PG-18-9. A/E shall follow PG-18-3 Design and Construction Procedures for applicable codes and construction procedures. Additionally, A/E shall incorporate HVAC Design Manual for New, Replacement, Addition and Renovation of Existing VA Facilities (March 2011). The above guides are available on the VA Technical Information Library located at http://www.cfm.va.gov/til/. The A/E will integrate the VA Sustainable Design Manual into design of space. GENERAL PROVISIONS AND DESIGN FIRM RESPONSIBILITIES The A/E will be given the appropriate orientation by the VA NIHCS offices of Engineering Service and VISN Contracting. The A/E shall prepare all necessary drawings. The degree of completion and the stages of submission shall be as hereinafter specified. A/E discussion with the VA NIHCS personnel concerning requirements will be arranged by and participated in by appropriate Engineering Service personnel. The A/E shall participate with the Project Engineering and other Engineering Services in review of project scope, design review comments, etc. A/E shall provide one (1) submission for schematic design drawings, one (1) submission for design development drawings, and two (2) submissions for construction documents (95% and 100% complete) plus a final submission which constitutes a complete bid package consisting of drawings, specifications, detailed final cost estimates, phasing and sequence drawings within each phase, and project bid alternates. Project will be designed using Building Information Modeling (BIM) as per the VA BIM guide located on the VA Technical Information Library located at http://www.cfm.va.gov/til/bim/BIMGuide/. The A/E shall develop the project utilizing AutoCADA(R)/RevitA(R) 2022. A/E shall include all libraries and families of the associated materials used on the project in the Revit file. The A/E will also provide an accurate rendering of the project in Revit at each design submittal in order to walk shareholders through the final product. This will be shown to shareholders during the design review meeting held at each phase of the design. For preparation of construction specifications, the A/E will provide specifications in "Word" format and Adobe Acrobat format. The A/E shall supply copies of the specification files, combined into a single document, to be electronically transported to the CO for incorporation into the VA specification booklet. The A/E firm shall provide a preliminary construction schedule which shall include a timeline for projected outages highlighting the facility interruption time frame for each affected area. This information shall be communicated to all bidders for their review and inclusion within their bids. It shall be communicated within the bid documents that the contractor will be required to review and provide any concerns regarding the projected construction timeline in order to minimize any/all disruptions to the medical center. All work shall be coordinated with the upcoming and current projects to prevent any duplication of efforts. A/E to build cost escalation into the construction probable cost estimate to account for delays in contracting and award. The A/E shall carefully review all requirements of the latest edition of the Accreditation Manual for Hospitals of the Joint Commission, with respect to design and operating requirements and report all conflicting conditions in writing. Specific service requirements to be discussed with applicable stakeholders during the initial design meeting and continuous collaboration between the VA, design consultant, commissioning agent and independent life safety reviewer throughout the project. PERIOD OF PERFORMANCE AND MILESTONES Each milestone in the timeline is subject to completion of the predecessor task and project requirements external to AE scope. The following is an estimated milestone schedule in terms of calendar days. Federal holidays do not count as calendar days. DESIGN PERIOD SERVICES The first submission is to be a 35% Schematic Submission (SC) and shall be submitted within 90 days after contract notice to proceed. This submission shall include: Construction drawing prints one (1) A 1/2 size (17x22) Preliminary digital cost estimate Digital drawing files Revit and PDF Preliminary digital renderings The second submission is to be a 65% Design Development Submission (DD) and shall be submitted within 75 days after VA approval and comments. This submission shall include: Construction drawing prints one (1) A 1/2 size (17x22) Construction drawing prints one full size (30x42) One (1) digital copy of preliminary master specifications in PDF separated by division. Digital revised cost estimate Digital drawing files Revit and PDF Digital Renderings The third submission is to be a 95% Construction Document Submission (CD) and shall be submitted within 45 days after VA approval and comments. This submission shall include: Digital copy Construction Statement of Work Construction drawing prints one full size (30x42) Digital copy of revised specifications Digital copy of refined cost estimate including deduct alternate and cost escalation Digital drawing files Revit and PDF Digital Renderings The fourth submission is to be a 100% Construction Document Submission and shall be submitted within 14 days after VA approval and comments on 100% submission This submission shall include all digital files: 1. Construction Statement of Work 2. Final specifications 3. Final cost estimate with deduct alternates 4. Drawing files Revit and PDF 5. Digital Renderings The final construction document bid set shall be submitted within 7 days after VA approval and comments.) This submission shall include both digital and hard copy: 1. Construction Statement of Work 2. Construction drawing prints: a. Two (2) sets A 1/2 size (17x22) b. Two (2) sets full size (30x42) 3. Final specifications two (2) hard copies 4. Final cost estimate with deduct alternates 5. Drawing files on Revit and PDF 6. Digital Renderings only CONSTRUCTION PERIOD SERVICES Contract Procurement Attend one (1) contractor's Pre-Bid site visit and respond in writing with recommendations to any questioned items. If necessary, develop amendments or assist with project procurement. Submittal Review The A/E firm shall review all construction materials and equipment and change orders proposed by the contractor for use in construction of this project. All submittals and change order proposals will be reviewed for compliance with the specifications and reasonable costs and returned to Engineering Service within 7 calendar days after receipt. The A/E firm shall review and respond to all RFI s within 5 business days. Site Visits (In addition to site visits during the pre-bid phase) Attend one (1) pre-construction meeting, after award of construction contract. Attend four (4) site inspections held at the 25%, 50%, 75%, and final inspection of project. Spot checking of contractor s progress based on two (2) 2-hour site visits when requested by the COR. These meetings may also be used to answer technical questions and/or develop minor changes due to site conditions, etc. The A/E shall attend a one hour weekly construction meeting for the duration of construction (via phone). The A/E shall visit the site at the request of the Contracting Officer to answer technical questions and/or provide clarification to documents due to errors or omissions at no cost to the government. These meetings will not count for meetings noted in items b or c above. As Built Drawings The A/E shall modify design drawings based on marked drawings supplied by the contractor and are to be verified by the A/E in field. Final drawings shall include all addenda and Bulletins issued by the VA and the A/E during construction. The changes shall be incorporated into the CAD files with the revision documented on the title block as appropriate with changes clouded. The A/E is to reformat the drawings based on the VA layering standards before final submission. These required layer standards will be provided under separate cover. Electronic .pdf and .dwg files of the drawings are to be submitted to the COR on CD. Any approved shop drawings with major differences from contract drawings, supplemental agreements processed change orders, etc., shall be incorporated. SUBMISSION REQUIREMENTS: Qualified SDVOSB A-E firms are required to respond if interested by submitting one (1) original completed Standard Form 330 qualification package Parts I and II to include all consultants (form available on-line at: http://www.gsa.gov/portal/forms/download/116486) In Section H of Part 1 include information regarding the firms estimating effectiveness (estimated costs vs. actual costs), an organizational chart of the firm, a design quality management plan, and past compliance with performance schedules (original design schedule vs. actual design completion time) for each key design project listed in Section F. All submissions shall be made electronically by email. Selection criteria shall be in accordance with Federal Acquisition Regulation (FAR) Part 36.602-1 as follows: 8. SELECTION CRITERIA. The government will evaluate each potential contractor under the terms of its: a. Professional qualifications necessary for satisfactory performance of required services; b. Specialized experience and technical competence in the type of work required, including, where appropriate, experience in energy conservation, pollution prevention, waste reduction, and the use of recovered materials; c. Capacity to accomplish the work in the required time; d. Past performance on contracts with Government agencies and private industry in terms of cost control, quality of work, and compliance with performance schedules; e. Location in the general geographical area of the project and knowledge of the locality of the project; provided, that application of this criterion leaves an appropriate number of qualified firms, given the nature and size of the project. 9. SUBMITTAL INSTRUCTIONS. All interested and capable contractors must submit an electronic copy of their SF330s, via email, by the closing time/date of this announcement. No hard/physical copies will be accepted. They must be submitted to the contract specialist at Kenyon.Dulaney@va.gov.
