Site work for a water / sewer project in Toledo, Ohio. Completed plans call for site work for a water / sewer project.
This project will install 433-feet of 8-inch diameter ductile iron pipe on Academy Avenue from Bancroft Street to Oxford Drive to replace and existing 4-inch water main. This project will install 189-feet of 8-inch ductile iron pipe on Oxford Drive from Academy Avenue to Lane Avenue.to replace existing 4-inch and 6-inch water main. This project will improve Academy Avenue through removal of existing wearing course and resurfacing. All bids must have the required paperwork and bid guaranty attached with this e-bid in order to be deemed responsive. If you are e-bidding and are using a check as the bid guaranty, the check must be received by the Division of Purchases and Supplies no later than the due date and time to be deemed responsive. The bid guaranty may be a bid bond or a certified check or cashier's check on a solvent bank. City forms must be filled out and attached to the bid: Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; MBE Good Faith and Goal Commitment; EEO Form; Suspension and Debarment; Primary and Subcontractor Forms; Best Bid Criteria; Signed Contract by Vendor CEO/President ; MBE Participation form; Certificates of Insurance; Current Worker's Compensation Certificate; Court-Ordered Child Support Affidavit; Independent Contractor PERS forms; and additional Insurance as Required. The following items will be required prior to awarding the contract: PLA paperwork is due upon award. Prevailing Wages are required for this project. It is the obligation of a contractor or subcontractor to make payment in accordance with the prevailing wage determinations.
