Renovation of a municipal facility in Cambridge, New York. Completed plans call for the renovation of a municipal facility.
The Capital District DDSO is seeking bids/quotes for the above referenced project (see attached specifications). If you are interested in bidding on this project, a SITE VISIT for this project is NOT MANDATORY. Please RSVP to PSESpurchasing@opwdd.ny.gov if you plan on attending. To allow time to sign in, it is recommended that you arrive to the site visit 10 minutes prior to the scheduled time. If this bid includes work completed by a sub-contractor, the sub-contractor bid form must also be completed. If the site visit is MANDATORY you must sign in at the site visit for your bid to be valid. Please write clearly for communication purposes and ensure your email address is clearly written. Contractors are encouraged to ask questions regarding the specifications of this project at the site visit. The purpose of asking questions during the site visit is to obtain clarification regarding the enclosed Project Scope. In the event that modifications are needed to the Project Scope, the modifications will be distributed by the CAPITAL DISTRICT DDSO Business Office to all contractors as an ADDENDA. Contractors are to prepare and submit their quotes based on the written Project Scope and any modifications made in writing through the ADDENDA. Prevailing Wages and Certified Payroll: NYS Labor Law requires that all contracted employees preforming maintenance, repairs or capital improvements in state owned buildings receive the prevailing wage and supplements rate for the occupation in which they are working. If this is a public work contract covered by Article 8 of the Labor Law or building service contract covered by Article 9 thereof, neither Contractor's employees nor employees of its subcontractors may be required or permitted to work more than the number of hours or days stated in said statutes, except as otherwise provided in the Labor Law as set nforth in prevailing wage and supplement schedules issued by the State Labor Department. Certified payroll record MUST be submitted at the time of invoicing. The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 30 days of award, unless modified below. This is a NYS Public Works project and prevailing wages are to be paid. Refer to attached specification for the following scope of work. Site must be cleaned, and all materials secured at the end of each workday. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of before removal of equipment and scaffolding,contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent. SCOPE OF WORK includes but is not necessarily limited to the following. 1. Facing the house, the tank is buried at the right rear corner by the chimney. 2. Contractor is responsible for contacting dig safe NY (811) before digging. UDIGNY.ORG 3. The excavating and removal of the buried fuel tank. (Unable to verify the tank capacity.) 4. If the tank was leaking or there is a fuel spill, contact DDSO staff and (Plant Superintendent 518-701-4111) for further instructions. DEC will also be contacted. 5. Remove fuel lines going into the house and plug the holes with a watertight product. SDS sheet is required for the product being used. 6. The hole where the tank was will be filled with clean fill, compacted, 4 inches of topsoil, with grass seed, and straw. 7. The removal of the fuel tank will follow all New York State code requirements along with any DEC requirements. Work is to be done between the hours of 8:00 am and 4 pm, Monday through Friday. Contractor is advised that this is an occupied home and may be occupied during these times. A. Lawn Damage/Property Damage o If the delivery of materials or any other work necessitates the contractor, his suppliers, or sub-contractors to drive on the lawn for ANY reason, the contractor shall be held responsible for the repair of the lawn. o Personal vehicles are not to be parked on the grass. o Any property damage caused by work performed is to be corrected to the condition existing prior to start of the job. B. Continuity of Work/Communication/Scheduling o Once the job has begun, it is to be worked on continuously until completed. o If there are any delays, even if due to inclement weather, a Work Control Representative MUST be contacted. o Work must be completed with in 7 days of receiving the purchase order.C. House Safety o The residence will be occupied during the job. o The contractor must take all reasonable care to safeguard the residents and the staff. o The contractor shall ensure all tools, materials and other items are picked up, removed, or stored and are safely secured at the end of each workday. o Any ladders, scaffolding or lifts used in conjunction with this job are to be stored to the satisfaction of the House Director at the end of the day. o The work as indicated above shall be scheduled as not to interfered with other functions of the residence. All work performed as part of this job is to be coordinated with the House Director and Plant Superintendent listed above.
