Construction Inc., awarded amount is $216,996.80.** Furnishing parking lot paving and striping at Ahukini State Recreational Pier, Lihue, Kauai, Hawaii, Department of Land and Natural Resources, Division of State Parks SCOPE OF WORK Contractor shall furnish all labor, equipment, tools, materials and transportation/mobilization to perform all operations in connections with providing parking lot paving and striping services at the Ahukini State Recreational Pier, Lihue, Kauai, Hawaii. The Department of Land and Natural Resources (DLNR) Division of State Parks will require the contractor to: 1. Install and maintain Best Management Practices (BMP) 2. 2" cold planing of the existing AC pavement = 2600 SY 3. Excavate gravel shoulder area approximately 6" deep at 625 SY 4. Recompact existing subgrade = 3225 SY 5. Install new aggregate base course 6" thick = 625 SY 6. 2" AC paving of State Mix V = 3225 SY 7. Install new parking lot markings to match existing ADA parking and existing parking lot Stalls. 8. Install new concrete parking wheelstops in front of the ADA parking and parking lot stalls =15 9. Install new concrete parking wheelstops spaced at 42" between each wheel stop from The break wall corner edge of the parking lot to the parking lot entrance. Wheelstops Shall be located 24" from the end of pavement. No striping required in this area. 10. Remove and properly dispose of any excess excavated/cold planing materials, gravel, And dirt from the job site. Ok to trim back Naupaka on the South Eastern end of the Parking lot to prepare appropriate ground work, BPM's, cold planing, compacting, and AC paving. CONTRACT ADMINISTRATOR For the purpose of this contract, David Tsuchiya, Kauai State Parks Superintendent of the Department of Land and Natural Resources, or designee, is the designated Contract Administrator (CA). Cell 808-346-3921 TERM OF CONTRACT The term of this contract shall be for a 1 time paving and striping of the parts and materials listed above. The contract shall begin once an approved purchase order has been received by the Department of Land and Natural Resources, Division of State Parks. Contract end shall be when payment has been received by the contractor from the State of Hawaii, Division of State Parks. OFFEROR QUALIFICATIONS Offeror must be (HCE) Hawaii Compliance Express certified. Offeror must also have a permanent office location in Hawaii where he/she will be accessible to telephone calls regarding complaints or requests that need immediate attention. Offeror shall have adequate equipment and number of employees to perform and complete the work specified herein within the period specified if awarded the contract. Offeror must possess and provide a Contractor A license. BID QUOTATION Unit bid price shall include labor, parts, materials, all applicable taxes and any other costs incurred to provide services specified above. GENERAL 1. All work must be done between 7:00 AM and 4:30 PM, Monday through Friday, except on State Holidays 2. The Contractor shall provide safety signs, barricades, and other devices necessary for the safety and convenience of the general public. The Contractor shall at all times conduct his/her work to assure the least possible disruption to normal public traffic (vehicular and pedestrian), allowing for the free flow of traffic and public right of ways. 3. Any damage to plants and other property or items in the area and adjoining properties shall be repaired and/or replaced by the contractor at his/her own expense and restored to the satisfaction of the Contract Administrator and the injured party(ies). SITE INSPECTION Prior to submittal of an offer, offerors may inspect the location to familiarize themselves with existing conditions, rules and regulations, and the extent and nature of work to be performed. Offeror inspection is not mandatory, however, submission of an offer shall be evidence that the offeror understands the scope of the project and shall comply with specifications herein, if awarded the contract. No additional compensation, subsequent to bid opening, shall be allowed by reason of any misunderstanding or error regarding site conditions or work to be performed.
