The Base Contract will generally include the full-width mill and overlay of 12 Township roadways (40,193 SY/3,705 TON). The Base Contract will also include 1,480 SY of pavement repair, 968 TON leveling/scratch course, the replacement of all pavement markings and the replacement of 60 LF of asphalt curb. Alternate/Add-On No. 1 will generally include the full-width mill and overlay of a portion of Arrowhead Drive (5,280 SY/475 TON). Alternate Add-On No. 1 will also include 250 SY of pavement repair, 140 TON of leveling/scratch course and the replacement of two stop bars. Alternate/Add-On No. 2 will generally include the full-width mill and overlay of a portion of South Poplar Road (2,630 SY/240 TON). Alternate Add-On No. 2 will also include 200 SY of pavement repair, 60 TON of leveling/scratch course, the replacement of two stop bars and one 6' x 30' crosswalk. Alternate/Add-On No. 3 will generally include the full-width mill and overlay of a portion of Buttonwood Drive (4,150 SY/380 TON). Alternate/Add-On No. 3 will also include 325 SY of pavement repair, 100 TON of leveling/scratch course and the replacement of one stop bar. Attachment #1 of PennDOT Proposal and Contract MS-944 contained in the Proposal and Contract Section of the Contract Documents, provides the quantities for each roadway. The "Instructions to Bidders" section describes the bidding requirements under this Contract. The project is subject to the terms of the Public Works Employment Verification Act (Act 127 of 2012). Completed and signed "Public Works Employment Verification Form" is required as a precondition to the award of the Contract and is to be submitted as part of the submission of the bid. This project is subject to the terms and conditions of the Pennsylvania Prevailing Wage Act, as determined by the Secretary of Labor and Industry and, therefore, must be bid as such. All bidders on this project must be pre-qualified by the Pennsylvania Department of Transportation in accordance with Section 102.01 of PennDOT Specifications Publication 408, latest edition. All bidders on the project must comply with the Township's Responsible Contractor's Ordinance 514 and 548 and complete the Contractor Responsibility Certification Form included in the Contract Documents. All work and materials shall be in accordance with PennDOT Standards and Specifications. Proposals must be accompanied by electronic copy of a certified check or bid bond in the amount of 10% of the total bid amount for the work detailed in Attachment 1. Proposals must also be accompanied by a completed Bid Form, Bidder's Qualification Statement, executed Anti-Collusion Affidavit and Public Works Verification Form as set forth in the Contract Documents. Failure to submit these three items with the bid proposal may result in disqualification of the bid.
