Each bidder is required to furnish with its proposal a Bid Guaranty and Contract Bond in accordance with Section 153.54 of the Ohio Revised Code. Bid security furnished in Bond form, shall be issued by a Surety Company or Corporation licensed in the State of Ohio to provide said surety. Each proposal must contain the full name of the party or parties submitting the proposal and all persons interested therein. Each bidder must submit evidence of its experiences on projects of similar size and complexity. See mandatory bid forms of the bid package. The owner intends and requires that this project be completed no later than 120 days from the issued Notice of Commencement date. All contractors and subcontractors involved with the project will to the extent practicable use Ohio products, materials, services, and labor in the implementation of their project. Additionally, contractor compliance with the equal employment opportunity requirements of Ohio Administrative Code Chapter 123 is required. This project is being funded in whole or in part by the Community Development Block Grant Program (CDBG). All federal CDBG requirements will apply to the contract. Bidders on this work will be required to comply with Section 109 and Executive Orders 11246 and 11375 which prohibit discrimination in employment regarding race, creed, color, sex, or national origin. Bidders must comply with Title VI of the Civil Rights Act of 1964, the Davis-Bacon Act and Wage Rates, the Anti-Kickback Act, and the Contract Work Hours and Safety Standards Act. Bidders must make positive efforts to use small and minority-owned businesses and exert good faith efforts to employ, train and offer contract opportunities in accordance with Section 3 of the Housing and Urban Development Act of 1968. Record Retention As the City of Lakewood, CDBG or the United States Government (USG) may legitimately request from time to time, the Contractor agrees to make available for inspection and/or reproduction by the agencies, all record, books, and documents of every kind and description that relate to this contract. Asphalt Resurfacing The work to be accomplished under this contract includes planing of the existing asphalt concrete pavement, full depth pavement repairs as needed, resurfacing with asphalt concrete, erosion control, installation of ADA curb ramps, pavement striping, full-depth concrete replacement, installation of new concrete stamped crosswalks, adjustments of manholes, catchbasins, monument boxes and valve boxes to grade, repair of catchbasins and manholes, and other appurtenances. Curbs and driveway aprons will be replaced as directed. Halstead and Lakewood Avenues streets are to be resurfaced with asphalt as shown on the project plans. o Concrete Removal and Replacement - Clifton Blvd/West 117 th Street intersection is a complete concrete removal and replacement and landscape improvement as shown on the project plans. A. Schedule All work shall be complete within 120 days of the issuance of the Notice of Commencement. Notice of Commencement is anticipated to be issued by June 15, 2023 or sooner. B. Engineer's Opinion of Probable Construction Costs The Engineer's Opinion of Probable Construction Costs is $1,000,000.
