Renovation of a day care center in Madison, Indiana. Completed plans call for the renovation of a day care center.

THIS PROJECT CONSISTS OF A LUMP SUM BID FOR THE RENOVATION OF AN EXISTING STRUCTURE, ARCHITECTURAL IMPROVEMENTS, MECHANICAL, ELECTRICAL, PLUMBING SYSTEMS, AN ELEVATOR, EXTERIOR IMPROVEMENTS, AND OTHER MISCELLANEOUS ITEMS. After review and award of a contract, the Owner will provide a "Notice to Proceed" to the Contractor. Contractor will then have 10 working days after said "Notice to Proceed" to commence construction and 60 days total to achieve substantial completion and an additional 30 days to complete all work for this project, for a total of 90 days after issuance of "Notice to Proceed" to project closeout. Substantial completion date is therefore anticipated to be SEPTEMBER 19, 2023 and final completion date is therefore anticipated to be OCTOBER 19, 2023, based on a Contract approval by JULY 5, 2023, and Notice to Proceed dated JULY 19, 2023. Contractor will be assessed $300.00 a day liquidated damages for any work remaining over and above either of the specified contract completion dates. Payment to be made on a standard monthly claim basis, with 10% retainage, and said retainage may be held up to three months after the successful completion of this contract. Copies of the Contract Documents are on file for review by appointment the CHILDRENS' ADVOCACY CENTER, 100 E 2nd Street, MADISON, INDIANA. Include specific project request, complete company info, address, phone numbers, and two email addresses for plan distribution. Only those plan holders registered through the Architect will be notified of addenda. Bidders on this work shall be required to comply with the provisions of the President's Executive Order No. 11246, as amended. The Bidders shall also comply with the requirements of 41 CFR Part 60 - 4 entitled Construction Contractors - Affirmative Action Requirements. A copy of 41 CFR Part 60 - 4 may be found in the Supplemental General Conditions of the Contract Documents and Specifications. The bidder's attention is also called to the "Minority/Women Business Participation" requirements contained in the Project Specifications. The State of Indiana, Office of Community and Rural Affairs, has adopted a State goal of 10% participation for minority and female owned businesses for construction related or purchase related contracts for the work. In addition, the Contractor must meet guidelines and appropriate Federal regulations including: 1) Executive Order 11246, 2) Section 3 of the Housing and Community Development Act of 1968, as amended, 3) Certification of Non- Segregated Facilities, 4) OMB Circular A-102, 5) Title VI of the Civil Rights Act of 1964, 6) Section 504, Rehabilitation Act of 1973, 7) Age Discrimination Act of 1975, 8) Executive Order 12138, 9) Conflict of Interest Clause, 10) Retention and Custodial Requirements for Records Clause, 11) Contractors and Subcontractors Certifications, and others that may be appropriate or necessary. In addition, the Contract Division procurement is subject to the Federal Regulations contained in the OMB Circular A-102, Sections B and O and the State of Indiana requirements contained in IC-36-1-9 and IC-36-1-12. If awarded, the Contract will be awarded to the lowest, most responsive, responsible bidder. Pursuant to Chapter 5, 5-4 of the Labor Standards Administration and Basic Enforcement Handbook 1344.1 Rev 2; "No contract may be awarded to any contractor that is debarred, suspended or otherwise ineligible to participate in Federal or Federally assisted contracts or programs. Any contract awarded to a prime contractor or subcontractor that is found to be ineligible for award must be terminated immediately." Prior to contract award prime contractors are to be actively registered or seeking registration with SAM.gov to determine eligibility/debarment status. In accordance with the Davis-Bacon requirement all contractors and subcontractors are required to pay not less than the prevailing wage rates listed on the federal wage determination for each classification of work performed on the project. The work to be performed and the bid to be submitted shall include sufficient and proper sums for all general construction, mechanical installation, labor, materials, permits, licenses, insurance, and so forth incidental to and required for the construction of the facilities. The successful bidder shall furnish a Performance and Labor and Materials Payment Bond in an amount equal to one hundred percent (100%) of the contract sum with an approved surety company. Bids must be submitted on the forms in the Contract Documents and other conditions therein described must be met. Each bid must be enclosed in a sealed envelope, clearly marked CHILDRENS ADVOCACY CENTER on the face of the envelope and display the name and address of the bidder. Each bid must be accompanied by a Bid Bond or Certified Check in a sum equal to 5% of the amount of the bid unless otherwise specified, and a completed Non-Collusion Affidavit. Bid prices must be firm for a period of THIRTY (30) days from the bid opening date. Should a successful bidder withdraw his bid, or fail to execute a satisfactory contract within ten (10) days after notice of acceptance of his bid, the owner may declare the Bid Security forfeited as liquidated damages, not as penalty. The successful bidder shall furnish a Performance and Labor and Materials Payment Bond in an amount equal to one hundred percent (100%) of the contract sum with an approved surety company. Said bond shall remain in full force and effect for a period of one (1) year after date of final acceptance of the work
