Site work, paving, new construction and renovation of a day care center in Fontana, California. Working plans call for the construction of a day care center; for the renovation of a day care center; for site work for a day care center; and for paving for a day care center.

City of Fontana is accepting Statements of Qualifications from qualified Design-Build Entities to provide Design-Build Services for a proposed project. The proposed project is located on the southwest corner of Arrow Blvd. and Tokay Ave. in the City of Fontana (APN 023217108; Reference Exhibit "A"- Vicinity Map) and will construct a one-of-a-kind community care center aimed to serve the homeless, those at threat of being homeless, and those experiencing medical and/or mental challenges. The project will be segmented into two phases and will include various amenities, site parking, offsite improvements, transitional housing, and prefabricated modular housing units and prefabricated administrative building. Reference Exhibit C - Preliminary Site Plan for the proposed site plan and phasing. The project will be designed and constructed in two Phases. Only Phase I will be performed under this solicitation. Phase I shall include the construction of Phase 1 as shown in Attachment 05- Preliminary Site Plan to include the parking lot, landscaping (Meeting MWELO requirements and Landscape Ordinance), lighting plans, pad foundations for the prefabricated housing units and administrative building, utility connections, fencing, monument sign, and any other construction required to complete this Phase. This phase will also include the construction of the offsite improvements for the entire project (Phase 1 and Phase 2) to include street improvements, traffic signal modification (Relocation of two traffic signal poles) for the intersection of Arrow Blvd. and Tokay Ave., sewer improvements (Sewer Lateral connection to existing sewer main), street lighting, signing & striping, fiber optic interconnect plans, WQMP, and any other construction required for such improvements. To be prequalified, DB-Entities must include a prime contractor who possesses a valid California General Building Contractor (B) license, prime contractor and/or subcontractor shall possess a Class "A" contractor's license or a combination of Class "C" specialty contractor's license(s) sufficient to perform the work, a design member who possesses a valid California Architect's license, and a design member who possesses a valid California Registered Civil or Structural Engineer's license. Upon final selection, a short list from qualified entities will be determined. The City will then issue a Request for Proposals and select a Design-Build Entity for the Project in accordance with Section 22164 et seq. of the California Public Contract Code. Funds from the Coronavirus State Fiscal Recovery Fund and/or the Coronavirus Local Fiscal Recovery Fund, together known as the Coronavirus State and Local Fiscal Recovery Funds ("CSLFRF") program, will be used to fund all or a portion of this Project.
