Proposal for a 12 month service maintenance agreement that includes 1 annual maintenance visit and 3 operational maintenance visits with a period of performance date starting I am issuing this solicitation to request a firm, fixed price proposal for a 12 month service maintenance agreement that includes 1 annual maintenance visit and 3 operational maintenance visits with a period of performance date starting August 1, 2023 - July 31, 2024that pertains to the attached equipment list spreadsheet for Cleaver Brooks 4WG-350HP models of fire tube boilers. This solicitation will include a base year plus 2 option years should you wish to quote the options years on the PUR 55 form. It is not required. The fire tube boilers references a steam safety risk which necessitates a safety plan.
