The City of Dubuque Water Department is requesting sealed bids for the purchase and delivery of Calcium Oxide (High Calcium Quicklime) for the remainder of CY 2023 through June 30, 2024, at which time the contract shall go to month-to-month with written agreeance from both parties until such time as the City were to re-bid. Documentation Requirements To facilitate proper evaluation, quotes must include the following: 1) Supplier's price per Ton of Calcium Oxide (FOB destination). The unit price must include all costs for supply, delivery, insurance, permits, testing and any other related fees. 2) Earliest delivery date, for partial or full batch. 3) A list of three (3) municipal water treatment plants (with responsible individual and telephone number) that are current customers of the supplier). 4) Copies of "Typical Chemical Analysis Reports" with average and typical range values for each parameter shown if available. 5) A copy of the most recent chemical test report. 6) A full description of the supplier's product quality assurances of its suitability as an additive for drinking water and ANSI/NSF 60 Certification information including a copy of the ANSI/NSF 60 Certification listing the end supplier as the certified agent.
