Renovation of a data center facility in Lawrenceburg, Indiana. Completed plans call for the renovation of a data center facility.

All Prime Contractors/First-Tier Contractors wishing to bid and contract for this project must be prequalified in order to participate in the bidding process and to be considered for an award of contract. The prequalification process will be administered by The City's Conconsidered for an award of contract. The prequalification process will be administered by The City's Construction Manager as Constructor (CMc), Maxwell Construction Company. The following are the bid package category items under consideration for this project. These may change Best and lawfully authorized to do business in the jurisdiction in which the Projects are located. oPlease note that it is a requirement for this job that any general liability and umbrella or excess liability policy have additional insured coverage that is primary and non-contributory, shall apply to both ongoing and completed operations, and shall provide coverage to the additional insureds (the Owner, Architect, and Construction Manager) for the full limits of such policies. Additionally, all insurance policies required shall be endorsed to include a waiver of subrogation in favor of the additional insured. By signing this prequalification form, the company agrees that if it is prequalified and its submitted bid is awarded the job, it will produce a certificate of insurance documents, with all supporting endorsements attached, that reflects the insurance requirements stated above. iProvide safety records with EMR rating for the past three (3) years along with a list of any OSHA violations within the past 10 years. iProvide a copy of your firm's current safety manual. iCertify your firm's drug testing plan is per Indiana Code. iProvide a description and history of your firm's efforts to maintain a diverse culture through recruiting, retaining, and hiring minority, women and veterans. iProvide history of any past and current litigation, arbitration, or mediation within the last 10 years. iProvide a list of Owners / Officers of your company along with contact information. iProvide a list of data center projects, completed within the past 5 years, along with reference contact information. iProvide a list of three Prime/General Contractors that you have worked under along with reference contact information. iProvide a list of three current significant projects presently under construction. iProvide an additional list of all projects that you have completed for this Owner within the last 10 years. iProvide a list of three supplier or manufacturer references along with reference contact information. iProvide history of any bankruptcy protection filed within the past five (5) years. iProvide company revenue for years 2019-2022. iProvide completed Financial Statements including Balance Sheet, Income Statement, and Opinion Letters for the last 2 years. iPrior to award of contracts Contractors will be required to complete the City of Lawrenceburg Contractor Safety Pre-qualification packet. iPrime Contractors/First-Tier Contractors may be found to be unqualified for these projects if the CMc determines that: (i) the above requirements have not been satisfied, (ii) a history of non-compliance by the Prime Contractor/First-Tier Contractor that has negatively impacted the schedule, budget, safety or quality of a project, (iii) if the Prime Contractors/First-Tier Contractor's experience on comparable projects is insufficient, (iv) the Prime Contractors/First-Tier Contractor's available manpower is insufficient based on its current workload, or (v) any of the required information provided is deemed unacceptable by the Owner or CMc.
