Furnish and Install for Synthetic Turf for Baseball/Softball Complex at the Branson RecPlex in Branson, Missouri The work to be performed under the provisions of this contract consists of the complete construction as follows: This project will consist of furnishing all labor and material for installation of synthetic turf and the improvements shown within these specifications. These improvements include removing existing ground cover and preparing ground cover for turnkey synthetic turf installation. Each proposer shall submit with their proposal a cashier's check, certified check or bidder's bond for 5 percent (5%) of the total amount of the base proposal made payable to the City of Branson, Missouri, hereinafter referred to as the Owner. Electronic bid bonds may be uploaded with the bid submission or submitted by using partners, Surety 2000 or Surety Wave. Cashier's check or certified check shall be submitted to address below and must be received prior to the bid opening date. Maddux, Ste 205 Branson, MO 65616 The bidder to whom a contract is awarded will be required to furnish a performance bond and a payment bond on the forms provided, acceptable to the City of Branson, Missouri, each in the amount of 100 percent (100%) of the contract price, in conformity with the requirements of the General Conditions. Wage rates paid on construction work for this project shall be at least equal to the prevailing wage rates as determined by the Division of Labor Standardsof Missouri. Enrollment in Federal Work Authorization Program Bidders are informed that pursuant to Section 285.530, RSMo, as a condition of the award of any contract in excess of five thousand dollars ($5,000.00), the successful bidder shall, by sworn affidavit and provision of documentation, affirm its enrollment and participation in a federal work authorization program with respect to the employees working in connection to the contracted services. Successful bidders shall also sign an affidavit affirming that it does not knowingly employ any person who is an unauthorized alien in connection with the contracted services. AffidavitforService Contracts The Bidderrepresents, in accordance with Sec. 285.530.2, RSMo that they have not employed, or subcontracted with, unauthorized aliens in connection with the scope of work to be done under the RFB and agrees to provide an affidavit to the City of Branson affirming that they have not, and will not in connection with the RFB, knowingly employ, or subcontract with, any person who is an unauthorized alien. Anti-Discrimination Against Israel The Bidder agrees to provide a certification if requested by the City of Branson, in accordance with Sec. 34.600, RSMo that they have not engaged in a boycott of: Goods or services from the State of Israel; Companies doing business in, or with, Israel; Companies authorized by, licensed by, or organized under, the laws of the State of Israel; or Persons or entities doing business in the State of Israel. Equal Employment Opportunity (EEO). A. The provisions of Article 15-A of the Executive Law and the rules and regulations promulgated thereunder pertaining to equal employment opportunities for minority group members and women shall apply to all Contractors, and any subcontractors, awarded a subcontract over $25,000 for labor, services, including legal, financial and other professional services, travel, supplies, equipment, materials, or any combination of the foregoing, to be performed for, or rendered or furnished to, the contracting State agency (the "Work") except where the Work is for the beneficial use of the Contractor. Safety Training Bidders are informed that the Project is subject to the requirements of Section 292.675, RSMo, which requires all contractors or subcontractors doing work on the Project to provide, and require its on-site employees to complete, a ten (10) hour course in construction safety and health approved by the Occupational Safety and Health Administration ("OSHA") or a similar program approved by the Missouri Department of Labor and Industrial Relations which is at least as stringent as an approved OSHA program. The training must be completed within sixty (60) days of the date work on the Project commences. On-site employees found on the worksite without documentation of the required training shall have twenty (20) days to produce such documentation. The Contractor and his subcontractors will be obligated not to discriminate in employment practices. Contract time will be specified in the Bid Form. No Bid may be withdrawn within a period of 90 days after the date fixed for opening Bids. The Owner reserves the right to reject any or all Bids, to waive informalities, and to reject nonconforming, nonresponsive, or conditional Bids
