Only contractors deemed as Qualified Contractors to provide Concrete and Portland Cement Concrete (PCC) Pavement Maintenance Services through the routine maintenance prequalification process are eligible to participate in this Invitation to Bid/Request for Quote. Through this ITB/RFQ, the Georgia Department of Transportation (hereinafter referenced as State Entity, Department, Agency, or GDOT) is seeking bids for PCC Slab Replacement. All respondents to this ITB/RFQ are subject to same terms and conditions of the MMSA 48400-410-0000033622, subsequent awarded contract, and the Invitation to Bid (ITB) / Request for Quote (RFQ) Form, and are cautioned to completely review the entire ITB/RFQ and follow instructions carefully. Project limits are from Mile Post 226 to Mile Post 228.49 North Bound and South Bound including all on and off ramps from I-75. Contractor shall furnish all labor, materials, equipment, means of transportation and incidentals necessary to perform work. Attached is a log and a list of approximate quantities and locations for estimating purposes only. It is the intent of this contract that all damaged slabs, cracks, and spalls be repaired within the project limits. This project shall be bid per Line Item.
