Criminal History background checks To protect residents of the Washington State Veterans Homes (WSVH) from contact with persons who have been convicted, or have a pending charge, of a crime or a negative action as defined in WAC 388-97 and WAC 388-113-0220, policy number 615.000 establishes standards for Washington Department of Veterans Affairs (WDVA) to conduct background checks for agency employees, applicants for employment, volunteers, work study, and contractors. All employees, applicants for employment, work studies, contractors, and volunteers who may have unsupervised access to vulnerable adults will be informed of the requirement for a background check and must provide written authorization for a background check using written authorization for a background check using DSHS Form 09-653 (Background Check Authorization). Failure to provide written authorization will disqualify the individual from employment and contracting with WDVA and Department Enterprise Services. All consultants, contractors and subcontractors working in WDVA campuses will adhere to Policy number 615.000 and its procedures.
