Site work for a civil project in Shelton, Connecticut. Completed plans call for site work for a road / highway.

To provide labor, materials, equipment and all else necessary for Pedestrian Signal at Long Hill and Constitution for the City of Shelton, CT 06484, based on the attached specifications. Notes: 1. Proposals are to be submitted in a sealed envelope and clearly marked "Bid # 2024-03" and "Pedestrian Signal at Long Hill and Constitution" on the outside of the envelope, including all outer packaging, such as DHL, FedEx, UPS, etc. 2. One original of the bid must be submitted; additional copies may be requested in the specifications. 3. It is the sole responsibility of the bidder to see that the bid is received by the City of Shelton Purchasing Department prior to the time and date noted above. Bids are not to be submitted via email or fax. 4. Bids may not be submitted with plastic binders or covers, nor may the bid contain any plastic inserts or pages. 5. The bid opening will be in room 200-B of Shelton City Hall at 2:00 p.m. on Thursday, August 17, 2023 and will comply with Covid-19 restrictions in place at the time. 6. The City of Shelton is an Affirmative Action / Equal Opportunity Employer. It is each Bidder's sole responsibility to monitor the above website for all updated information; addenda will not be mailed, e-mailed or faxed out. Questions concerning this specifications and bidding procedures are to be submitted in writing to Sgt. Mark Siglinger, Shelton Police Department, mark.siglinger@sheltonpolice.org. Written requests for information will not be accepted after 10:00 a.m. on August 9, 2023. Verbal requests for information will not be accepted. Failure to comply with these conditions will result in the bidder waiving the right to dispute bid specifications and conditions, no exceptions. A response in the form of an addendum will be posted to the City's website no later than 5 p.m. on August 11, 2023; it is the bidder's sole responsibility to monitor the website for updated information about this bid. Any contact about this bid between a Bidder and any other City official and/or department manager and/or City of Shelton employee, other than as set forth above, may be grounds for disqualification of that Bidder. No questions or clarifications shall be answered by phone, in person, or in any other manner other than specified above. Requirements Any sizes or estimates of quantities as shown on drawings or stated herein are approximate and are not guaranteed in any respect. The City reserves the right at all times to increase or decrease the amount of work if deemed in its best interest. Price is to include all labor, materials, tools, equipment, plant, mobilization, permits, insurances, etc., required to properly complete the project. The City of Shelton reserves the right to award the bid with multiple items: o To more than one bidder, based on meeting the item(s) specification, cost, availability, or any combination of these criteria; o To a single bidder who meets the specifications for all items, and offers the best combination of lowest cost, best availability, and broadest product range; o And may add, subtract, or delete any item and/or quantity as deemed in the best interest of the City. The Bidder must not discriminate, nor permit discrimination, against any person on the grounds of race, color, national origin, religion, sex, handicap, or veteran status, in their employment practices, in any of their contractual arrangements, in all service and accommodations they offer to the public, and in any of their other business operations. The successful bidder MUST secure all required permits (local, state, federal) prior to commencing work on the site. Scope of Work / Specifications Work shall include the following: 1. Labor, materials and equipment necessary for the complete installation of pedestal equipment for one crossing on Long Hill Avenue at the intersection of Constitution Blvd. 2. Labor, materials and equipment necessary for the complete replacement of the loop detectors on Long Hill Avenue and Constitution Blvd. 3. Mobilization and de-mobilization 4. Two pre-cast foundations 5. Wiring, conduit and paving of conduit 6. New controller 7. Modification of cabinet for pedestal phase 8. Pedestal Equipment for one crossing 9. All electrical and programming 10. Curb cuts for handicap access into the crossing 11. Restoration of all disturbed areas 12. Traffic Control when and where deemed necessary by the Shelton Police Department Bid Requirements If necessary: Please provide the following information with your bid: Qualifications and relevant licenses Experience: List of similar projects Client list: List clients for similar projects, including names and contact information Personnel Qualifications and Resumes Insurance Requirements Experience Modification Rating: The bidder's Workers Compensation Experience Modification Rating (EMR) must be 1.0 or lower. Bidders with an EMR higher than 1.0 are permitted to submit a bid for this project, however, the City of Shelton reserves the right to reject any or all bids form bidders with an EMR higher than 1.0. The 1.0 EMR limitation also applies to all lower tier trade subcontractors for this project. All individuals assigned to the Project site may be required to undergo a background check by Shelton Police Department (SPD). If required, the name and date of the individual must be furnished to the Contractor at least seven (7) calendar days before the individual is to arrive on site. Individuals who ahe not been approved by the SPD will not be allowed on site. Individuals who are convicted sex offenders, who have committed crimes against minors, or who have been convicted of a serious felony are not permitted on the Project. Individuals with other offenses will be evaluated by the SPD on a case-by-case basis. NO FIREARMS OR CONTROLLED SUBSTANCES ARE PERMITTED ON CITY PROPERTY, REGARLDESS IF THE INDIVIDUAL HAS A VALID PERMIT TO CARRY / USE. Insurance Requirements The Respondent shall, at its own expense and cost, obtain and keep in force during the entire duration of the Project or Work the following insurance coverages covering the Respondent and all of its agents, employees and sub-contractors and other providers of services and shall name the City of Shelton, its employees and agents as an Additional Insured on a primary and non-contributory basis to the Respondent's Commercial General Liability and Automobile Liability policies. These requirements shall be clearly stated in the remarks section on the Respondent' s Certificate of Insurance. Insurance shall be written with insurance carriers approved in the State of Connecticut and with a minimum Best's Rating of A-VIII with all policies written on an occurrence form basis. In addition, all carriers are subject to approval by the City. Minimum Limits and requirements stated below: 1) Worker's Compensation Insurance: - Statutory Coverage - Employer's Liability: $500,000 each accident / $500,000 disease-policy limit / $500,000 disease each employee - A Waiver of Subrogation shall be provided in favor of the City, their employees and agents. 2) Commercial General Liability: - Including Premises & Operations, Products and Completed Operations, Personal and Advertising Injury, Contractual Liability and Independent Contractors. - Limits of Liability for Bodily Injury and Building Damage Each Occurrence $1,000,000 Aggregate $2,000,000 (The Aggregate Limit shall apply separately to each job.) - A Waiver of Subrogation shall be provided in favor of the City, their employees and agents. 3) Automobile Insurance: - Including all owned, hired, borrowed and non-owned vehicles - Evidence of Combined Single Limit of Liability for Bodily Injury and Building Damage: Per Accident $1,000,000 A Waiver of Subrogation shall be provided in favor of the City, their employees and agents. The respondent shall provide a Certificate of Insurance as "evidence" of General Liability, Auto Liability including all owned, hired, borrowed and non-owned vehicles, statutory Worker's Compensation and Employer's Liability and Professional Services Liability coverage. The respondent shall direct its Insurer to provide a Certificate of Insurance to the City before any work is performed. The awarded Respondent(s) will be responsible to provide written notice to the Owner 30 days prior to cancellation or non-renewal of any insurance policy. The Certificate shall evidence all required coverages including the Additional Insured on the General Liability and Auto Liability policies and Waiver of Subrogation on the General Liability policy. The respondent shall provide the City copies of any such insurance policies upon request.
