The AHA is soliciting and accepting proposals from qualified, licensed, and insured consultants or companies, demonstrating their qualifications, past performance and interest for this work. The term Proposer used herein shall mean proposers, partnerships, corporations, associations, or professional organizations. All Proposers are asked to describe the tasks required to successfully carry out either one or both of the Scope of Services outlined in Exhibit D, attached hereto. However, Proposers may include additional services that the Proposer is capable of providing and which, in the Proposers opinion, would enhance the implementation of the proposed Scope of Services. Proposers must provide pricing for any additional services presented in the proposal. Pricing for all five (5) years must be included in the proposal, including any increases, broken down by year. The AHA anticipates that it will initially award a contract for the period of three (3) years with the option, at the AHAs discretion, of two (2) additional one-year option periods, for a maximum total of five (5) years.
