Demolition and site work for a playground / park / athletic field in Lake Charles, Louisiana. Completed plans call for the demolition of a playground / park / athletic field; and for site work for a playground / park / athletic field.

Preference is given to materials, supplies, and provisions that are produced, manufactured, or grown in Louisiana, quality being equal to articles offered by competitors outside the State. All bids must be accompanied by bid security equal to five percent (5%) of the sum of the base bid and all alternates, and must be in the form of a certified check or cashiers check drawn on a bank insured by the FDIC, or a Calcasieu Parish Sheriffs Office Bid Bond Form contained in contract documents, shall be written by a surety or insurance company currently on the U.S. Department of the Treasury Financial Management Service list of approved bonding companies which is published annually in the Federal Register, or by a Louisiana domiciled insurance company with at least an A- rating in the latest printing of the A.M. Bests Key Rating Guide to write individual bonds up to ten percent of policyholders surplus as shown in the A.M. Bests Key Rating Guide or by an insurance company in good standing licensed to write bid bonds which is either domiciled in Louisiana or owned by Louisiana residents, all in accordance with LSA--R.S. 38:2218. No Bid Bond indicating an obligation of less than five percent (5%) by any method is acceptable. If bid is submitted electronically, an electronic bid bond as described in LSA--R.S. 38:2212 E.(1) per the standards adopted by the office of the governor, division of administration and the office of the information technology as provided for in LAC 4:XV.701 will be required. The successful bidder shall be required to furnish a Performance and Payment Bond in an amount equal to 100% of the Contract amount, shall be written by a surety or insurance company currently on the U.S. Department of the Treasury Financial Management Service list of approved bonding companies which is published annually in the Federal Register, or by a Louisiana domiciled insurance company with at least an Arating in the latest printing of the A.M. Bests Key Rating Guide to write individual bonds up to ten percent of policyholders surplus as shown in the A.M. Bests Key Rating Guide or by an insurance company in good standing licensed to write bid bonds which is either domiciled in Louisiana or owned by Louisiana residents, all in accordance with LSA--R.S. 38:2219. The public shall incur no obligation to the Contractor until the Contract between the Calcasieu Parish Sheriffs Office and the Contractor is fully executed. The Sheriff Office reserves the right to reject any and all bids for just cause as permitted by LA R.S. 38:2214B. The ability of an Entity to reject any bid is applicable only when administered in accordance with the Public Bid Law. In accordance with LSA--R.S. 38:2212(B)(1), the provisions and requirements of this Section, those stated in the bidding documents shall not be waived by any entity. Bids may be held by the Sheriffs Office for a period not to exceed forty-five (45) days from the date of the opening of bids for the purpose of reviewing the bids and investigation the qualifications of bidders, prior to award. Official action on this bid will be taken within Forty-five (45) days by the Calcasieu Parish Sheriffs Office. SITE WORK - REMOVE LIMESTONE SURFACING WHERE DESIGNATED. o REMOVE GEOTEXTILE FABRIC. o REMOVE GEOGRID. o ACCESS TO SITE AND DUMPSTER PAD HAS MIN. 10" LIMESTONE THICKNESS, AND OTHER ROADWAY AND PARKING PAD AREAS HAS 6" THICKNESS. o REMOVE 4 FT WIDE X 4 INCH THICKNESS LIMESTONE WALKWAY FROM EACH PARKING PAD TO TRAILER STEPS. - REMOVE (2) 18" RIBBED PVC CULVERTS FOR CURRENT DRIVEWAY ACCESS. - REGRADE THE SITE o TO BE PAID AS EXCAVATION AND EMBANKMENT BY THE LUMP SUM o GRADE SITE TO PREVIOUS CONDITION AS NOTED IN PLANS (SEE TOPOGRAPHIC SURVEY SHEETS). o TOPSOIL IS CURRENTLY STOCKPILED TO THE SOUTH OF THE BORROW PIT. CONTRACTOR SHALL EVENLY SPREAD THIS TOPSOIL ACROSS THE ENTIRE DISTURBED AREA OF THE SITE. FINISHED GRADE FOLLOWING PLACEMENT OF TOPSOIL SHALL BE THE ELEVATION/CONTOURS OF THE PROVIDED TOPOGRAPHIC SURVEY AS STATED IN THE PLANS. NO DIRECT PAYMENT WILL BE MADE FOR SOILS HAULED IN TO GRADE SITE. WATER - WATER SERVICE IS SUPPLIED BY CITY OF LAKE CHARLES WATER DIVISION. - THE 6" PVC WATER SUPPLY TIES IN TO EXISTING WATER MAIN (SIZE NOT IDENTIFIED) NORTH OF THE SITE ALONG JAMES SUDDETH PKWY. STARTING FROM THE LOCATION OF THE TIE-IN, THE 6" SUPPLY (BELOW GRADE UP TO THE LOCATION OF THE SITE'S ACCESS DRIVEWAY) SHALL BE REMOVED. A FIRE HYDRANT WITH 6" LEAD IS LOCATED HERE AND SHALL ALSO BE REMOVED. - THE WATER SUPPLY LINE REDUCES TO 4" PVC AFTER THE FIRE HYDRANT. THE SERVICE METER AND GATE VALVE ADJACENT TO THE SITE ACCESS DRIVEWAY SHALL BE REMOVED, AND SHALL BE RETURNED TO THE CALCASIEU PARISH SHERIFF'S OFFICE. CONTRACTOR SHALL REMOVE THE 4" PVC WATER LINE THAT CONTINUES BELOW GRADE UNTIL AFTER THE 2ND ROADWAY CROSSING. A RISER INSTALLED AT THIS LOCATION BRINGS THE WATER LINE ABOVE GRADE. -REMOVE THE 4" PVC WATER SUPPLY LOCATED ABOVE GRADE WITHIN THE UTILITY CORRIDOR. - PIPE PLUG SPECIFICATIONS AND DRAWINGS TO BE PROVIDED FOR APPROVAL, PRIOR TO WORK BEING PERFORMED. SEWER - SEWER SERVICE IS PROVIDED BY CITY OF LAKE CHARLES WASTEWATER DIVISION. - AN 8" PVC SEWER SERVICE THAT TIES IN TO EXISTING 12" PVC SEWER LINE NORTH OF THE SITE ALONG JAMES SUDDETH PKWY WHERE THE EXISTING SEWER MAIN TERMINATES SHALL BE REMOVED. TWO SEWER MANHOLES SHALL BE REMOVED AS PART OF THIS WORK. - PIPE PLUG SPECIFICATIONS AND DRAWINGS TO BE PROVIDED FOR APPROVAL, PRIOR TO WORK BEING PERFORMED. ELECTRIC - TO BE PERFORMED BY OTHERS MISC - INSTALL EROSION CONTROL MEASURES IN ACCORDANCE WITH EROSION CONTROL SITE PLANS A AND B.
