Renovation of a mixed-use development in Decatur, Georgia. Completed plans call for the renovation of a 13,860-square-foot medical facility.

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Request for Quote (RFQ) and the solicitation number is 36C24E23Q0144. The government anticipates awarding a firm-fixed price contract resulting from this combined synopsis/solicitation. This solicitation is issued as a 100% set aside for small business concerns. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov. STATEMENT OF NEED (SON) CONTRACT DESCRIPTION AND REQUIREMENTS The Atlanta VA Medical Center has a requirement to resurfacing and sealing the Veterinary Medical Unit (VMU) Floor. Vendor must also need to move cage equipment, racks, feed and bedding, and various machinery to complete task. A firm-fixed price contract will be awarded for the purchase of services in accordance with all terms, conditions, provisions, specifications and schedule of this solicitation herein. Quotes shall contain the Offeror s best terms for cost/price and technical capabilities of the required services. The government reserves the right to award without discussions. 1.1 PRICING The Offeror shall submit price quotes for the required services to the specifications listed in Section 1.2 below. Please enclose the established price list and description of capabilities and product specifications for the requirement. Items offered that are brand name or equal shall identify items as being on GSA schedule and reference the contract number. Note: The Offeror should include a description of any support services in the technical quote. ITEM INFORMATION ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 1.00 EA __________________ __________________ The vendor shall supply all labor, materials, and equipment as necessary to complete the following tasks in accordance with the Statement of Work (SOW). The Vendor shall service the entire VMU, including hallways, procedure rooms, cage wash area, and laboratories (breakroom not included) approximately 13,860 SF. The project will be completed in 4 sections, to allow for continued operations. Floors must be available to be utilized within 8 hours of installation. Work should be complete within 8 weeks from start date. PRINCIPAL NAICS CODE: 238330 - Flooring Contractors PRODUCT/SERVICE CODE: Z2DZ - Repair or Alteration of Other Hospital Buildings GRAND TOTAL __________________ 1.2 GENERAL INFORMATION STATEMENT OF WORK A. GENERAL INFORMATION 1. Title of Project: Veterinary Medical Unit (VMU) Flooring Resurfacing and Sealing of Work: This requirement is a service contract for resurfacing and sealing the VMU Floor. Vendor must also need to move cage equipment, racks, feed and bedding, and various machinery to complete task. Background: The VMU s aging flooring is allowing leaks to administrative offices below into lower 3rd floor. The flooring can no longer meet the regulatory requirements for sealed, impermeable surfaces in vivaria due to cracks, gaps, and chips. Floor must be sealed. A service contract is needed to ensure that timely repairs are made so sanitation can be maintained. Description of Tasks and Associated Deliverables: The vendor shall supply all labor, materials, and equipment as necessary to complete the following tasks in accordance with the Statement of Work (SOW). The Vendor shall service the entire VMU, including hallways, procedure rooms, cage wash area, and laboratories (breakroom not included) approximately 13,860 SF (see Attachment 2 Floor Plan). The project will be completed in 4 sections, to allow for continued operations. Floors must be available to be utilized within 8 hours of installation. Task One: Move equipment and furniture to storage area Move cage equipment, racks, feed and bedding, and various machinery to designated storage area (TBD) before servicing floor. Move all equipment, racks, feed and bedding, and various machinery back after completion of floor servicing Task Two: Mechanically prepare substrate for new flooring system and dispose of debris. Proper preparation of all new and existing surfaces in a satisfactory manner to receive new finishes. This includes: The demolition and removal of necessary items Testing the concrete slab to confirm that conditions meet requirements, including moisture; if not, apply a moisture mitigating primer Preparation or fill of slab joints, existing or new, on an as-needed basis Touch-up (leveling, sloping, and patching of up to 15 golf ball size spalls/holes per 1,000 square feet) and/or refinish of surfaces damaged by subsequent work Existing slope to drains followed unless discussed prior All work shall be performed in conformance with manufacturer s installation methods Task Three: Furnish and install flooring finish, with 6-inch integral cove base. The installer must be authorized by the manufacturer, and be a certified Society of Protective Coatings Application Specialist All animal procedure rooms (imaging, ORs, treatment rooms, etc.), small animal holding, and necropsy: heavy-duty urethane and epoxy mortar flooring system with integral base or urethane climatic resinous floor with an integral base Large animal holding and cagewash sterilizer spaces: urethane climatic resinous floor with an integral base The final coat of high wear urethane must have chemical resistance to withstand the sanitation and disinfecting solutions used within a pharmaceutical or related environment. All work shall be performed in conformance with manufacturer s installation methods 6. Type of Contract: Firm Fixed Price 7. Place of : Veterinary Medical Unit Atlanta VA Medical Center 1670 Clairmont Road Decatur, GA 30033 The evaluation of quotes and the as to the equality of the products shall be the responsibility of the Government and shall be based on information provided by the Offeror. The Government is not responsible for locating or securing any information not provided in the quotation by the Offeror. To ensure that sufficient information is available, the Offeror must furnish, as a part of the quote, all descriptive material necessary for the purchasing activity to determine whether the products meet the salient characteristics of this requirement. Vendors proposing to modify a product to make it conform to the requirements of this announcement shall include a clear description of the proposed modification and clearly mark any descriptive material to show the proposed modification. Offers that do not have descriptive material will not be considered for award. Instructions to the Offeror Basis for award resulting from this RFQ will made to the Best Value Offer that provides a complete pursuant to the preparation requirements set forth in 52.212-1. FAR 52.212-2 (Evaluation- Commercial Items) will be incorporated in full text into the RFQ and will be evaluated on the following factors. Evaluations will be in accordance with FAR 13 Simplified procedures. Evaluation Factors for Award Factor 1 - Technical Capabilities All offerors must meet or exceed all requirements and salient features as listed in the Statement of Need. Factor 2 - Past - Past will be based on the most current PPIPRS Report. Factor 3 - Price The following FAR provision and clauses apply to this solicitation and are incorporated by reference. The solicitation document, incorporated provision and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-89, Effective July 2, 2015. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquistion.gov.
