The completion of Form 96 requires that a Bidders Financial Statement be included with bid submission. The Bidder should be certain to complete Form No. 96, Part II, Section II, Plan and Equipment Questionnaire. THIS INFORMATION IS MANDATORY FOR A VALID BID. These four questions are: A. Have you explained your plan or layout for performing proposed work B. If you intend to sublet any portion of the work, state the name and address of each subcontractor, equipment to be used by the subcontractor, and whether you expect to require a bond. C. What equipment do you intend to use for the proposed project D. Have you made contracts or received offers for all materials within prices used in preparing your proposal Other Considerations include: 4. All other forms needed must be completely and carefully filled for the bid to be accepted. 5. Consult with the Owner about the work schedule or the completion of this bid package. 6. Please find Supplemental Bid Form in the last page of this Specification. 7. Have you properly and completely executed Indiana Form No. 96 8. Have you enclosed bid bond or certified check in an amount equal to five percent (5%) of the bid This is mandatory. (Company checks are not acceptable). 9. Have you marked the character of the bid and the due date on the envelope
