Brandy Candelaria Assistant Director 702-651-4361 Brandy.candelaria@csn.edu PURPOSE OF REQUEST CSN invites interested parties to submit a Proposal for annual elevator maintenance and repair services. Services shall consist of, but are not limited to, furnishing all material, labor, tools, transportation and equipment necessary to provide full preventative maintenance service, lubrication, adjusting, cleaning, repairs, testing, reports, parts and trouble call service on all Vertical Transportation Equipment. See Exhibit 1 for list of locations and equipment to be maintained and serviced. CONTRACTOR'S DUTIES This Scope of Services establishes the minimum requirements for a full preventative maintenance program on all Vertical Transportation Equipment on each three of the CSN Properties located at North Las Vegas Campus, 3200 E Cheyenne Avenue, North Las Vegas, NV 89030, West Charleston Campus, 6375 W. Charleston Blvd, Las Vegas, NV 89146, and Henderson Campus 700 College Drive, Henderson, NV 89002. See Exhibit 1 for Elevator Equipment and Property Details. The Basic Services to be performed by Contractor under the scope of services shall consist of, but are not limited to, furnishing all material, labor, tools, transportation and equipment necessary to provide full preventative maintenance service, lubrication, adjusting, cleaning, repairs, testing, reports, parts and trouble call service on the Vertical Transportation Equipment listed on the Property Location (Page 33) Contractor certifies that it has examined or shall examine all Units of Vertical Transportation Equipment at the Properties described in the scope of work to be entered under the Contract, and is satisfied (or will be satisfied at the time of execution of Contract, with the condition of the Vertical Transportation Equipment to be maintained and any other conditions affecting the Basic Services to be performed. All Vertical Transportation Equipment maintained by Contractor is or will be accepted by Contractor in its "as is" condition. Any Basic Services not specifically mentioned in this RFP, but which are needed to make the maintenance complete within the intent of this Contract shall be performed at no additional cost to CSN. A survey was conducted and there are no outstanding repair found at this point in time. Contractor shall perform, coordinate and complete the requirements of the Scope of Work, in cooperation with any other contractors or trades then doing work on any Property (ies), promptly, diligently, and in a professional workmanship like manner to the full and complete satisfaction and acceptance of CSN. Contractor shall perform the requirements of the Scope of Work in a manner that will not impede or obstruct the ongoing business activities of any tenants or residents at the Property(ies). Contractor shall have a member of its supervisory personnel regularly engaged in inspection and supervision visit each Unit at least semi-annually to observe the quality of maintenance and to make certain that the quality of maintenance meets the specified and intended standards of this Scope of Services. The Supervisor shall meet with the management of their respective CSN property, via face-to-face meetings to discuss issues, repairs and trouble calls. The Supervisor shall also schedule within 30 days of the semi-annual visit a face to face meeting with CSN and will provide a written report, per this Scope of Services. Contractor agrees that, in the event of any vertical transportation equipment-related accident or problem of any kind, such as an injury to any employee or the general public, fire, unwanted fire alarm/detector activation, stuck elevator, entrapment, major mechanical damage/failure during equipment servicing, Contractor will immediately notify CSN and thereafter furnish a full written report of such accident to CSN within ten (10) business days of such occurrence.
