All bids must include 2 copies (1 original, 1 copy) of the Bid / Quote Form (Section 004126), the Familial Disclosure Affidavit (Section 004205) and the Iran Linked business Affidavit (Section 004210). Cut sheets for proposed furniture selections are to be submitted with bids. The bidder will execute the contract, provide the required insurance certificate(s) within the (10) days after notice of award of Contract but prior to Work commencing. Do not include the cost of Bond in Bid. Lakeview Public School District reserves the right to reject any or all bids received, to waive any informalities and irregularities in the bidding, and to accept a bid other than the lowest bid. 1.
