Quarterly preventative maintenance (PM) agreement which includes PM inspection of the following: A. Model 533LS Sterilization 21" B. GTS30AMOP3203 Generator C. HS-300 Watts, RO Water System D. Check calibration utilizing the PACS 3000 control system Details: 1. Technician must be trained on all the above-mentioned system components. 2. The technician will call to the attention of the customer any need for replacement parts. If replacement part is needed and available in customer's inventory, the technician will install needed parts. Necessary parts not in the customer's inventory will be ordered with customer approval for installation at the next scheduled inspection. 3. All labor for equipment inspection, adjustment, lubrication, installation of replacement parts and testing during scheduled PM inspections will be included in the offer price. 4. All labor for repair calls between PM inspections which are the result of equipment failure will also be included. 5. The Hawaii Department of Agriculture and the company receiving the award may extend the agreement for an additional (12) months at the same cost provided that both parties agree to the extension. 6.
