Contractor shall provide all necessary materials, supplies, tools, equipment, transportation, licenses, permits, prevailing wages, insurance, bonds, and personnel to provide the DSH with West Canteen Roof Replacement services. Question Deadline 10/19/2023 at 2:00 PM PT Bidders shall notify the DSH immediately via e-mail at jasmine.mckenzie@dsh.ca.gov and carbon copy aubreana.diaz@dsh.ca.gov if they need clarification about the services being sought or have questions about the IFB instructions or requirements. A. Description of Services/Background: 1. Contractor shall provide all necessary materials, supplies, tools, equipment, transportation, licenses, permits, prevailing wages, insurance, bonds, and personnel to provide the DSH with West Canteen Roof Replacement services. 2. The DSH-Patton is a forensic psychiatric hospital located in Patton, CA, in San Bernardino County. DSH-Patton was established in 1890 and opened in 1893. DSH-Patton provides treatment to forensically and civilly committed patients within a secure treatment area. The hospital does not accept voluntary admissions. 3. Please carefully review and consider the minimum qualifications, and the detailed Scope of Work located in Attachment 18, Sample Agreement, Exhibit A, Scope of Work, in order to complete your bid. B. Agreement Term and Security Provisions: 1. The term of any agreement resulting from this IFB is anticipated to be November 15, 2023, through June 30, 2025. Any agreement resulting from this IFB shall have no force or effect until it is signed by both parties and approved by the Department of General Services (DGS), if such approval is required. Performance shall start no later than on the expressed date set by the awarding agency and after all approvals have been obtained and the agreement is fully executed. 2. All performance, under the agreement, shall be completed on or before the termination date of the agreement. 3. Should the Contractor fail to commence work at the agreed upon time, the DSH reserves the right to terminate the agreement upon five (5) days written notice to the Contractor. 4. Bidder shall agree to all security provisions where the performance of work takes place on any state hospital grounds. Bidders shall notify the DSH immediately via e-mail at jasmine.mckenzie@dsh.ca.gov and carbon copy aubreana.diaz@dsh.ca.gov if they need clarification about the services being sought or have questions about the IFB instructions or requirements. The level of detail that shall be provided in response is subject to the availability of DSH resources. The DSH shall respond to all questions via an addendum to the solicitation with all questions and answers posted on Cal e-Procure Contractor shall provide all equipment, materials, supplies, personnel, transportation, hauling, trucks, disposing, dumpsters, testing services, licenses, labor, supervision, tools, safety gear, properly equipped vehicles, insurance and permits necessary to install a new shingle roofing system for the West Canteen Building. B. Replacement of West Canteen roofing: i. Contractor shall remove and dispose of existing comp shingle roofing system. ii. Contractor shall inspect existing decking, flashings, and roof penetrations etc. Replace or repair where necessary for damage or rot. Decking shall be replaced with CDX plywood as needed. If damage or rot exceeds an area equal to or greater than 50% of the total project scope, the decking shall be replaced in its entirety. iii. Contractor shall install 30 lb. roofing felt underlayment using manufacturer recommended installation methods. iv. Contractor shall install approximately one hundred fifteen (115) lineal feet of 2" metal drip edge. Contractor to submit material / color options to DSH-Patton Project Manager prior to material purchase and install. v. Contractor shall install one-layer new GAF Brand starter shingles, followed by new GAF Brand High Wind Class G shingles using six (6) nails per shingle and approximately thirty-five (35) lineal feet of GAF Brand High-Profile Ridge Cap according to manufacturer recommended installation methods. Contractor to submit shop drawings and material / color options to DSH Patton Project Manager prior to material purchase and install. vi. Contractor shall install one (1) new roof jack using industry standard installation methods. Roof jack shall be painted to match color of roof
