Furnish quarterly maintenance services and trouble call maintenance of restroom fixtures at Daniel K. Inouye International Airport. To be eligible for award, bidders shall possess a valid State of Hawaii Specialty Contractors C-37 license at the time of bidding. The scope of work consists of quarterly maintenance services and trouble call maintenance of restroom fixtures at Daniel K. Inouye International Airport on an" as-needed basis. SCOPE OF WORK - The work required to be performed by the Contractor shall consist of the furnishing of all labor, supervision, materials, supplies, equipment, and incidentals necessary to maintain restroom fixtures including quarterly servicing of soap dispenser and faucet/hand dryer combined systems at the Daniel K. Inouye International Airport. Work shall include the replacement of parts as necessary to maintain good operating conditions of restrooms. Exhibit A - Restroom location map and summary of quarterly maintenance work. Exhibit B - Soap dispenser and faucet manufacturer parts and product information ONE (1) YEAR period from the date indicated in the Notice to Proceed from the Department of Transportation. At the discretion of the State, the contract may be extended for three (3) additional ONE (1) YEAR periods not to exceed FOUR (4) YEARS per Section 10.6 of the project specifications. The State reserves the right to reject any or all proposals and to waive any defects in said proposals for the best interest of the public. To be eligible for award, bidders shall possess a valid State of Hawaii Specialty Contractors C-37 license at the time of bidding All Request for Information (RFI) questions and substitution requests shall be Maintenance of Restroom Fixtures Daniel K. Inouye International Airport Notice to Bidders Project No. BO1425-73 NTB-2submitted via HIePRO no later than October 23, 2023 at 2:00 p.m., HST. RFI questions received after the stated deadline will not be addressed. Verbal RFIs will not receive a response. All responses to RFI questions shall be issued by formal addendum and posted in HIePRO. The Equal Employment Opportunity Regulations of the Secretary of Labor implementing Executive Order 11246, as amended, shall be complied with on this project. The U.S. Department of Transportation Regulation entitled "Nondiscrimination in Federally-Assisted Programs of the U.S. Department of Transportation," Title 49, Code of Federal Regulations (CFR), Part 21 is applicable to this project. Bidders are here by notified that the Hawaii Department of Transportation will affirmatively ensure that the contract entered into pursuant to this advertisement will be awarded to the lowest responsible bidder without discrimination on the grounds of race, color, national origin or sex (as directedby23 CFR Part 200). TERM OF CONTRACT - The term of this contract shall be for a period of one (1) year from the date of the official Notice to Proceed issued by the State, unless renewed as specified in Subsection. LIQUIDATED DAMAGES - Timely performance by the Contractor of all of his duties every calendar day is an essential part of this contract and in case of failure on the part of the Contractor to perform any of his duties in a timely manner, damages will be sustained by the State. As the amount of damages are difficult to predetermine, the amount of such damages are fixed in advance as follows: $50.00 per calendar day for each incomplete unit area scheduled but not completed within 24 hours of the specified interval until the work is satisfactorily completed. The contractor shall notify the State if there any conditions preventing the completion of services in a timely manner. AOA SECURITY REQUIREMENTS The Contractor shall obtain AOA and Customs and Border Patrol (CBP) security badge and maintain annually. All costs for badging and security access shall be considered incidental to the contract work. PROPOSAL GUARANTY - A proposal guaranty (bid bond) is not required except when specifically noted in the proposal section of the bid document. When a proposal guaranty is required with a bid, it will be specifically stated in the proposal; and no proposal totaling $25,000 or more will be considered unless accompanied by one of the following forms of bidder's security: A. a deposit of legal tender; or B. a surety bid bond underwritten by a company licensed to issue bonds in the State of Hawaii and submitted on the standard form provided herewith; or C. a certificate of deposit, share certificate, cashier's check, treasurer's check, teller's check, or official check drawn by, or a certified check accepted by and payable on demand to the State by a bank, savings institution, or credit union insured by the Federal Deposit Insurance Corporation (FDIC) or the National Credit Union Administration (NCUA). 1. The bidder may use these instruments only to a maximum of one hundred thousand dollars ($100,000.00). 2. If the required security or bond amount totals over one hundred thousand dollars ($100,000.00), more than one instrument not exceeding one hundred thousand dollars ($100,000.00) each and issued by different financial institutions shall be acceptable. 3. The instrument shall be made payable at sight to the Department of Transportation, State of Hawaii. According to Section 103D-323, HRS, the above shall be in a sum not less than five percent (5%) of the amount bid.
