Renovation of a military facility in Waterbury, Connecticut. Completed plans call for the renovation of a military facility.

Grace Congregate Housing Corporation ("GCHC") is seeking a qualified firm to serve as Construction Manager for its proposed renovation of Grace House (the "Project"). GCHC is requesting a "Statement of Qualifications and Response to Request for Proposals" from interested parties for the Project. The Project is anticipated to be funded primarily with funding from the Department of Economic & Community Development Community Investment Fund, with additional ARPA funding from the Waterbury Community Services Investment Program (WCSIP). Prior successful experience working with US Department of Housing and Urban Development (HUD) multifamily affordable housing development rehabilitation projects funded by federal grants is mandatory to qualify for consideration for the Project. Project Summary Grace House is a forty (40) unit affordable housing development serving residents age 62 and older. The four-story building is comprised of all onebedroom apartments and is serviced by an elevator. The property is located at 25 Abbott Terrace, Waterbury. The proposed rehabilitation includes work related to the exterior building and building units. The capital improvements proposed include four distinct areas of the community: the building roof, the building exterior masonry, the installation of new energyefficient windows, and replacement of kitchen cabinets, countertops, and sinks in all residential units. The building has a flat roof system which is original to the building and is currently failing, allowing water infiltration in severe weather. The proposed roof replacement includes all areas of the entire flat main roof, the addition of the two elevator tops, and the stairwell roof. The scope of work includes replacement of all roof decking, installation of a new EPDM roof with a 20-year warranty, and foam insulation. The scope of work for the building exterior masonry is to address the failing vertical separation joints as well as the horizontal joints that exist on the building. It includes removal of all existing masonry joints and installation of new joints with water resistant backers and fillers. The complete power washing of the building to address areas of staining and replacement of any failed brickwork. The scope of work for the window replacements includes replacement of all building windows utilizing the existing metal frames. The new windows will be vinyl double hung windows in white as a retrofit window which avoids damage to the building stucco. The final project includes the replacement of the cabinets, countertops, and sinks in all residential units. The renovations will provide residents with adaptable kitchens which can easily be retrofit for accommodations due to aging and provide residents with an improved environment for both food preparation and storage. Preliminary construction cost estimates, based upon initial third-party quotes, were approximately $1 million. Scope of Services The CM shall review the need for construction documents with and participate in the design of such documents as needed with the GCHC. The CM shall solicit trade bids on behalf of the Owner from trade subcontractors on a competitive basis following DECD & City of Waterbury's required procedures related to procurement, wage rate and CHRO's requirements. The GCHC and the CM shall agree upon a Guaranteed Maximum Price to perform the work identified in the documents, and the CM shall enter into contracts with trade subcontractors to perform their respective trade work. The CM shall provide services including, but not limited to: Pre-construction Phase Scope of Services: 1. Constructability Reviews; 2. Site Logistics Plan; 3. Building Evacuation Plan; 4. Schedule and Phasing Coordination; 5. Cost Control Management; and 6. Construction Documents Conversion into Subcontractor Bid Packages. Bid-Phase Services: 1. Develop the Master Project ("CPM") Schedule; 2. Bid each Bid Package, public advertising bids (ex: State Department of Administrative Services ["DAS"] Contracting Portal); 3. Issue subcontractor (trade) bid packages as required by CHRO; 4. Conduct preconstruction conferences and site visits; 5. Process all addenda; 6. Receive bids from subcontractors and conduct public bid opening; 7. Issue Guaranteed Maximum Price; and 8. Execute subcontractor agreements. Construction Phase Scope of Services: 1. Comply with General Conditions; 2. Comply with General Requirements; 3. Conduct Pre-construction Conference; 4. Periodic update the Master CPM Schedule; 5. Monthly update of Schedule of values; 6. Review and Prepare Monthly Progress Payment Requests (in accordance with DECD & City of Waterbury's requirements); 7. Periodic Update of Project Cash Flow Projections; 8. Act as the Project's Prime Liaison; 9. Coordinate subcontractors' Requests for Information ("RFI-s") and architectural/engineering ("A/E") Requests for Information ("RFI") Responses; 10.Coordinate all change requests and responses; 11.Coordinate all types of submittals; 12.Coordinate all types of testing and Inspections; 13.Coordinate all sub-contractors; 14.Coordinate all sub-contractors participating in commissioning; 15.Provide construction trailers, storage, equipment, barriers, and etc.; 16.Provide all necessary on-site CM personnel; 17.Coordinate Substantial Completion and Turn Over; and 18.Closeout the Project It shall be the CM's responsibility to be familiar with, and take into consideration when responding to, this RFP, all of DECD's and the City of Waterbury's requirements including, but not limited to: construction contract requirements, warranty for construction defects, assurance of completion, progress payment requests, construction guidelines (including but not limited to: project planning & technical review, construction costs, energy conservation, environmental & hazardous material review), standards of design, cost certification, affirmative action, substantial completion/permission to occupy and final closeout.
