The Contractor must supply all the labor, materials, and equipment to include installation and site work for concrete slab at the following locations as specified within the NFC drawings (Attachment-A) for each site location. Camp Salmen Nature Park in Slidell, LA. Tammany Trace at Koop Drive in Mandeville, LA. Site Prep- Contractor to cut and remove a minimum of 6" of soil and organic matter, compact and grade, install 4-6" of gravel base material, install moisture barrier, install reinforcing steel (as specified in drawing), and finish concrete . All excavated materials and construction debris are to be removed and disposed of by contractor. The installation of all anchor bolts, fitness tile flooring, and fitness equipment is not included in this scope of work and will be installed by others. Any work that may be reasonably inferred from the specifications or drawings as being required to produce the intended result shall be supplied whether or not it is specifically called for. Work, materials or equipment described herein which so applied to this project are covered by a well known technical meaning or specification shall be deemed to be governed by such recognized standards unless specifically excluded. All work must be performed in accordance with the appropriate codes and comply with the Federal contract provisions contained in Attachment H- Federal Clauses (ARAP & HUD). The Parish reserves the right to revise this schedule. Any such revision will be formalized by the issuance of an addendum to the Quote Request. Question Deadline 11/14/2023 at 2:00 PM CT Any questions regarding insurance requirements please contact the Risk Department at 985-898-5226 or email riskman@stpgov.org If the estimated cost of the work is $50k or more a Contractor licensed by the State to do the type of work indicated in Section 2.1 may submit a quote. The Respondent's signature on the quote certifies that he holds an active license under the provisions of Chapter 24 of the Louisiana Revised Statutes, Title 37. Failure to be properly licensed constitutes authority for the Parish to reject the quote. The Parish reserves the right to require a Contractor licensed for less than $50k depending on the requirements set forth in Section 2.1 and shall provide within 10 days after being notified of an Award. For projects costing twenty-five thousand dollars ($25,000) or more, the successful Respondent shall be required to provide a performance (surety) bond in the amount of one hundred percent (100%) of the project cost to insure the successful performance under the terms and conditions of the contract negotiated between the successful Respondent and the Parish. In case the work is not completed in every respect within the time that may be extended, it is understood and agreed that per diem deductions of the sum of $500.00 for liquidated damages,
