NSU Optometry Flat Roof Question Deadline 12/04/2023 Any RFI's must be submitted in writing 72 hours prior to the bid opening. All questions shall be furnished in writing to: Northeastern State University Attention: Chris Gay gayc@nsuok.edu Purchasing Department 600 North Grand Avenue, Tahlequah, OK 74464 Site Directions: Administration building, at 600 N. Grand Ave., Tahlequah Ok. 74464 The bid shall be enclosed in a sealed envelope plainly marked and addressed as follows: The name and address of the bidder shall appear in the upper left-hand corner of the envelope. The lower left hand corner of the envelope shall be marked: BID PACKAGE: #B24-01- NSU Optometry Flat Roof. The envelope shall be addressed to: Northeastern State University Attention: Chris Gay Purchasing Department 600 North Grand Avenue Tahlequah, OK 74464 The following Contract (Bid Group) Work Summaries describe the intended scope of work to be included in the Bid Package. The bid shall include all the requirements of the plans with specifications unless specifically indicated otherwise in this summary. This includes all layout and daily clean up involved with the work. It is the bidder's responsibility to review bidding requirements and forms to insure a complete, safe, and coordinated execution of the Work Package being quoted. Additionally, bidders shall include in their pricing applicable portions of related specification sections normally associated with the work to be done whether specifically indicated by this Contract Work Summary or not. Should any bidder be unclear as to the scope of work required, he is instructed to contact the Owner for resolution prior to bidding. In the absence of written instruction to the contrary, in matters of specification and/or scope of interpretation, bidders shall include the more costly and complete of the possible interpretations. Awarding of Contracts will be based on best qualified bid. CONTRACT: General Construction Work including but not limited to the following: 1) Drawings and Specifications as identified in Contract Attachment "B" Including But Not Limited To: 1) NSU is a "Tobacco Free" campus; Contractor will be expected to abide by NSU policy and will be held accountable for all Subcontractors on site. 2) Contractor shall be responsible for coordination of all necessary local, state and federal, pre, during and post compliance inspections. Contractor shall provide NSU with documentation of compliance at completion of work. 3) Contractor to be responsible for all permits and fees associated with this scope of work. 4) Work may be halted and resumed at any given time according to NSU's needs. 5) Contractor shall be responsible for mobilization and staging locations to minimize daily operations of the university. 6) Utility terminations shall be in accordance with the governing utility procedures and regulations. NSU will be given a minimum of 24 hours' notice prior to any utility outages/shut-offs. 7) Contractor shall be responsible for coordinating any and all utility locations for the project. 8) Contractor is responsible for furnishing, receiving, protecting and installing specified materials per the construction documents. 9) Contractor is responsible for all field verifications of dimensions and conditions. 10) NSU will provide information on electrical panel locations and available power from each location. 11) Contractor shall replace damaged decking with 5/8" plywood and replace all "H" clips, as needed. (a unit price line for decking replacement is on the bid form) 12) Contractor shall install new water and ice barrier at all valleys, ridges, penetrations and eaves. 13) Contractor shall install new 30# felt underlayment. 14) Contractor shall install a new Architectural 30 year shingle. Color is similar to Tamko Heritage Shingle "Antique Slate". Owner shall select color from contractor provided submittals. 15) Contractor shall install new flashing at all vertical walls and penetrations. 16) Contractor shall install new drip edges at all perimeters. 17) Contractor shall remove and reinstall all guttering and downspouts necessary to complete roof installation. Gutters damaged during removal shall be replaced by contractor at contractor's expense. 18) Any rotten or damaged Fascia board shall be replaced by unit price on bid form. 19) All materials will be submitted to the owner for approval prior to installation. 20) Contractor shall provide a 20 year manufacturer warranty. 21) Contractor shall submit documentation as a manufacturer's certified installer prior to beginning work. 22) Contractor is to ensure that construction trash and demolished debris is gathered and disposed of daily in the contractor provided trash receptacle. 23) Contractor is responsible for daily clean-up and equipment storage. NSU is not responsible for damaged or stolen property and/or equipment. 24) Contractor shall be responsible for abiding OSHA and NSU safety regulations. 25) Contractor shall be responsible for protection of existing equipment and materials that are not called out to be demolished or repaired. 26) Any discrepancy between the plans/specifications and actual field conditions must be submitted in writing to the owner a minimum of 72 hours prior to the bid closing date. 27) Contractor shall be responsible for demolition, removal, loading and haul off of all roof materials, scraps and debris. 28) Contractor shall be responsible for repairing rutted or furrowed greenspace, by means of fill dirt and sod. 29) All demolished/removed materials and debris, unless otherwise specified, shall become property of the roofing contractor; and as such, shall be removed from site by the contractor, daily. 30) Contractor shall sweep the entire area with a rolling construction magnet daily to ensure nails and other small metallic debris are removed from the construction site. 31) Contractor shall utilize the parking and laydown area specified in the plans. Other green space or open areas shall not be utilized for the re-roofing process, unless pre-approved by the owner. 32) Contractor shall provide and erect fencing and or barricades around the wing or area that is being addressed. Barricades shall be mobile type with caution tape/flagging as needed. 33) Dumpsters, trash chutes, lifts or other construction equipment may be utilized, but must be kept or operated within the fenced area. 34) Contractor shall not interfere with building exits or egress paths of any occupied buildings. 35) Contractor shall access all roofs by exterior means only. Roof hatches will not be utilized. 36) Building access MUST be coordinated with Owner before entry. 37) Contractor shall be responsible for ALL trash and debris on the roof and along the perimeter of the building at ground level. 38) Contractor shall be responsible for temporarily waterproofing the building in the event the demolished portion is not recovered by the specified roof system at the end of the work day. 39) Contractor shall maintain a site supervisor at all times when the contractor has a presence on campus, including all sub-contractors to the awarded contractor. 40) Contractor shall be responsible for and acknowledge all plans, specifications, and details as shown in all bidding documents. 41) Contractor shall install a roofing system and abide by NSU's General Conditions in this and all other contract documents. 42) Construction schedule is to be between May 15th and July 31st 2017. 43) All material deliveries are to be addressed to the awarded contractor to be delivered at job site physical address. (Job site address: 113 Redman Drive, Tahlequah, OK 74464) 44) Contractor shall include a company portfolio, showcasing recent completed projects of similar size and design. A current W9 shall also be submitted with the completed bid documents.
