Renovation of a medical facility in Norman, Oklahoma. Completed plans call for the renovation of a medical facility.
The project consists of removal of all existing condensing units on ground level and roof top and all associated piping and electrical connections back to the source of power, removal of all air handling units in mechanical rooms and in ceiling spaces and all associated piping and electrical connections back to the source of power, portions of duct work as identified on plans, portions of ceiling tiles and ceiling grid system in the affected areas, capping existing incoming steam piping at entry point into the building, installing new condensing units on ground level including new refrigeration piping system, installing air handling units in ceiling spaces throughout the building including duct work as indicated on plans, installing electrical panels and feeders as per plans and specifications, replacing ceiling tiles and grid system in affected areas (contractor may reuse existing ceiling grid and tiles if maintained in same conditions as existing), furnishing and installation of all control wiring, thermostats, required interface control wiring, interfacing new HVAC control system with "EarthSmart" controls front end to accommodate monitoring of HVAC system operating status, performing complete test and balance to ensure proper system operation Description CAP-000803 - Building 54 HVAC System Replacement - Norman, OK - GMH - ODMHSAS List your company name, address, telephone number, Tax ID/FEI number, and email address on the bid form where indicated.* Complete and sign the bid affidavit. If any of the questions on the affidavit do not apply to you, enter "NONE" in the blank.* Fill in all blanks on the bid forms, including acknowledgment of addenda, base or total bid price, all alternate or unit prices. Use words and numbers where required.* Deliver to the Owner - Construction and Properties (CAP) proper bid security as required by the Bid Documents. (Required if bid is more than $50,000). Bid Bonds must contain original signatures and have a "Power of Attorney" attached. Copies of Bid Bonds are not acceptable. Checks must be either a certified or a cashier's check. If an Irrevocable Bid Letter of Credit is used, it must be complete, and on the form provided by the Owner.* Check the Online Plan Room linked from the Construction and Properties' website at https://omes.ok.gov/services/construction-and-properties/, then navigate to "Download project bid documents and submit online bids" under "Drawings" at least 24 hours in advance of the bid closing to verify that you have received all of the addenda, which have been issued.* General. 1.1 The undersigned, being familiar with the local conditions affecting the cost and performance of the work, and with the Contract Documents, including the Solicitation for Bids Notice, General Conditions, Special Conditions for Construction Contracts, Specifications, Plans and Addendum Number(s) (listed above) on file at the Owner's office (State of Oklahoma OMES/CAM/DRES Construction and Properties) Will Rogers Building, 2401 N. Lincoln Blvd. Suite 212, Oklahoma City, Oklahoma 73105-4402, and in accordance with the provisions thereof, hereby proposes to furnish all labor, materials and equipment necessary for the following, in accordance with the plans and specifications for CAP Project Number referenced above, for the sums listed below. 1.2 In submitting the bid, it is understood that the right is reserved by the State of Oklahoma to reject any and all bids, and it is agreed that this bid may not be withdrawn for a period of thirty (30) days after the date of closing of same. Work is to start within ten (10) days after receipt of Notice to Proceed/WORK ORDER. 1.3 If the bid exceeds $50,000, it shall be accompanied by a certified check or a cashier's check made payable to the State of Oklahoma, or a Corporate Surety Bond of a surety company duly authorized to do business in the State of Oklahoma, in the sum of five percent (5%) of the total amount of the bid (unless otherwise specified in the bid documents) which is submitted as bid security, conditioned upon the Bidder's entering into a contract with the State of Oklahoma in accordance with the terms of the bid. It is agreed that said bid security of the successful bidder will constitute liquidated damages, and not a penalty for the failure of the bidder to enter into a contract in accordance with this bid. 1.4 We propose to complete this work within 180 days from the date of receipt of the Notice to Proceed/WORK ORDER. Compliance. 2.1 The Bidder certifies that: 2.1.1 They are an Equal Employment Opportunity Employer and that they do not discriminate in any of their business or employment practices; 2.1.2 They, and all sub-contractors and suppliers performing work on the Project, will comply with the provisions of the Oklahoma Taxpayer and Citizen Protection Act of 2007 and participate in the Status Verification System. The Status Verification System is defined in the Oklahoma Statutes, Title 25 1312; 2.1.3 They will comply with the laws relating to public construction in the Oklahoma Statutes (Title 61) and the Oklahoma Administrative Code (Section 260, Chapter 65); and 2.1.4 They will comply with all State of Oklahoma Governor's Executive Orders, including those relating to the prohibited use of any and all tobacco product on any and all properties owned, leased or contracted for use by the State of Oklahoma, including but not limited to all buildings, land and vehicles owned, leased or contracted for use by agencies or instrumentalities of the State of Oklahoma.
