Minority-Owned Business Enterprise goal of 20% Female-Owned Business Enterprise goal of 10% ACE Business Concerns: Construction Contracts goal of 10% Non-Construction Contracts goal of 3% SCOPE OF WORK Cuyahoga Metropolitan Housing Authority (CMHA) is seeking proposals from qualified elevator maintenance and repair contractors to provide elevator preventive maintenance and repair services. CMHA intends to enter into a two-year contract with three one-year renewable options. Any increase in cost for each of the option years must be clearly identified by the responder in the proposal. The following specifications are intended to provide a detailed description of the requirements for a preventative maintenance/service contract for the equipment as listed. These specifications shall become a part of any final contract as if typed therein. The Owner shall have the right to inspect the equipment and services provided at all times. The proposer must provide current proof of insurance and licensing requirements. The proposer shall provide the following certificates evidencing the coverage amounts: • Workers’ Compensation & Employer’s Liability • General Liability • Auto Liability Contractor shall maintain, at its sole expense, throughout the term of the Agreement the following insurance: • Commercial General Liability Insurance with limits of $1,000,000 per occurrence and $2,000,000 aggregate for bodily injury, property damage, and personal & advertising injury. The policy must extend coverage to CMHA for the respondent’s Completed Operations. • Automobile Liability Insurance in an amount not less than $1,000,000 Combined Single Limit per accident. • Workers’ Compensation Insurance which meets statutory requirements and Employer’s Liability Insurance in the amount of $500,000 each accident and $500,000 each disease.
