The Alachua County Housing Authority (ACHA)is soliciting quotes from licensed Asbestos Removal companies and/or individuals to remove and clean up flooring that has non-friable asbestos mastic. The ACHA is looking to sign a one-year contract with the awarded contractor for services through an on call as needed bases. This will be an Indefinite delivery Indefinite Quantity contract with an option to extend for up to four more additional one-year periods. ACHA is not responsible for delays in submission. Anticipated date of Approval Wednesday , December 6, 2023 @ 5.00PM EST. I. Purpose It is the intent of this Request for Quote to obtain one asbestos removal company and/or individual to remove and clean up VCT and/or Ceramic flooring that has non-friable asbestos mastic underneath. 2. Scope of Work 2.1 The Awardee shall be responsible for: 2. 1 . 1 Securing the unit per required protocol 2.1.2 Removal and disposal of all VCT and/or ceramic floor tile throughout the unit 2. 1 .3 Removal of all mastic, glue, grout, etc. 2.1.4 Cleaning of all surfaces free of asbestos 2.1.5 Disposal of all removed materials and hazardous supplies 3. Terms and Conditions 3. The Awardee agrees to comply with the terms and conditions as specified in HUD Forms as Attached: 3./.1 Table 5.1 of HUD Procurement Handbook 7560. 1 3. 1.2 From HUD 5370-C Section II General Conditions for Non-Construction Contracts (Maintenance Contracts between $2,000 and $250,000) 3. 1.3 Form HUD 52158 Maintenance Wage Rate Determination 3.1.4 Legally Required Statement Regarding Access to Records 4. Special Conditions 4.1. Inspection: An ACHA representative will inspect all work prior to payment 4.2 Change Orders: ACHA may make Change Orders adding or deleting work as necessary if it is in the best interest of the Authority to do so. Both parties shall make no modifications of the terms of the contract except by written amendment or change order. Contractor shall not perform extra work unless authorized to do so in writing. 4.3 Invoicing/Payment: Pay requests are be sent to Ron Hall via email at Ron@acha-fl.com for work completed and inspected to date. 4.4 Sub-Contracting: Contractor agrees not to hire any sub-contractor without prior approval and clearance. 4.5 Insurance: Contractor agrees to provide proof of Insurance as follows: 4.5.1 Automobile Liability Insurance on owned and non-owned motor vehicles used in connection with this contract and shall contain a limit of bodily injury of at least $500,000 per person and a total limit per occurrence of at least $1,000,000 and ACHA.
