Renovation of a mixed-use development in Clinton, Oklahoma. Completed plans call for the renovation of a medical facility.

This is a combined synopsis/solicitation for items prepared in accordance with FAR Part 13. This announcement constitutes the only solicitation; s are being requested and a written solicitation will not be issued. The solicitation is issued as Request for s (RFQ) 246-24-Q-0007. This procurement is 100% buy Indian - under NAICS code 238210 with a size standard in millions of dollars $19.0. The Contractor shall provide and install a new wired and auxiliary IP-based wireless nurse call system and remove the old system components throughout the Clinton Indian Health center 10321 N 2274 Rd, Clinton, OK 73601 in accordance with the statement of work, specifications and floor plans. The period of for this requirement is 90 calendar days from Notice to Proceed. The official is November 29, 2023, 10:30 AM at the Clinton Indian Health Center. Questions are due no later than December 8, 2023 5pm CST. The closing date for receipt of s is December 29, 2023 COB CST. The offer shall be received by email to jeffrey.earnest@ihs.gov or in a sealed envelope, addressed to this office, showing the solicitation number, your name and address, no later than 5:00 p.m. CST, on December 29, 2023. CLAUSES INCORPORATED BY FULL TEXT: Please note that due to character limitation in FedBizOpps, full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulations (HHSAR) can be accessed on the Internet at http://www.acquisition.gov and acquisition.gov/hhsar FAR 52.213-4 Terms and Conditions-Simplified Acquisitions (Other Than Commercial Items)(DEC 2022). THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING: A) Firm, Fixed price with attached bid sheet B) Technically specifications of the statement of work, acceptable materials list to be used. C) (2) Relevant past of this type of work, shall provide organization's name, point of contact, summary of project and award amount D) Bid bond form (SF24) attached is required at $35,000. If over $35,000 and selected as the acceptable vendor proposal, (SF25) and payment (SF25A) bonds shall apply Offers shall be evaluated on an acceptable/non-acceptable basis. This process shall proceed until a technical acceptable is identified THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS IN THE STATEMENT OF WORK CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR OFFERING THE LOWEST REASONABLE PRICE AND ALL TECHNICAL SPECIFICATIONS. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions. **Please note that potential awardees may be required to pass a background investigation from the IHS Human Resource Department. This process may take up to two weeks ** Forms: OF 306 and Addendum to OF 306 (attached) This project will consist of the following: 1. Install new conduit distribution system from the power supply/controller units to the dome lights outside each room served by the system as shown on plans. Refer to Drawing A-2 for Existing and New s. 2. Pull new cable to dome lights outside each room served by the system. 3. As rooms are turned over to the Contractor, interface the existing conduit within each room to replace the cable and devices within that room while ensuring the existing Nurse Call system that is still being used in rooms yet not converted remain in operation. 4. Replace existing devices, controllers, master stations, power supplies, etc. and add new devices as identified on the plans to meet Code and other Owner requirements based on current usage of spaces. 5. Remove existing cabling, conduit, and other appurtenances when new system is fully functional. 6. The phasing of this project will be critical, as the Clinton Indian Health Center is required to provide at least 7-days of notice to patients and staff for limitations on scheduled care. Dates and durations of work will be established by the contractor.
