The Program Work Plan with Preventative Maintenance Tasks, Equipment List, Schedule Matrix, etc. is located in Appendix D of the Contract Boilerplate. Interested Vendors should review the Contract Boilerplate and IFB document for further information. Included with these documents is a Bid Quote Sheet in Excel format that should be used to submit your bid. The equipment in Building 60, as noted in the Appendix D, Program Work Plan, Equipment List - Exhibit B, is currently covered under a separate contract through January 31. 2026. The equipment will begin coverage under the terms of this contract effective February 1, 2026. Contract term: 10/1/24 - 9/30/29 The OMH Revite Team will perform the routine preventive maintenance of the Cabinet Unit Heater & Air Curtain equipment under this task as indicated in Appendix D, Exhibit B - Equipment List. The equipment is covered under this Contract for Time & Material (T&M) Repairs and/or Supplemental Preventive Maintenance coverage upon Facility request. The CONTRACTOR will provide a unit cost for this equipment for Supplement Preventive Maintenance in the event the FACILITY requests that the CONTRACTOR begin performing the preventive maintenance. Please note the addition of a One-Time Deep Cleaning and Refurbishment of the AHUs in Bldg. 40 included in the scope of work located in Appendix D, Exhibit A. Minority and Women Business Enterprises (MWBE) Participation Goals for this contract have been set at: MBE - 16%; WBE - 14%. Service Disabled Veteran Owned Businesses (SVDOB) Participation Goals for this contract have been set at 6% New York State Department of Labor Prevailing Wage Rates apply to this contract. "Pursuant to the New York State Finance Law Sections 139-j and 139-k, the Procurement Lobbying Act applies to this solicitation. The restricted period began with the publication of this ad and will end with the formal approval by OSC of the contractual agreement" Service-Disabled Veteran-Owned Set Aside: No Minority Owned Sub-Contracting Goal: 16% Women Owned Sub-Contracting Goal: 14% Service-Disabled Veteran-Owned Business Contracting Goal: 6% Questions from bidders attending the site visit are to be submitted by Wednesday, December 20, by 3:00 PM EST per the instructions in the IFB. Questions and Answers will be sent to all bidders attending the site visit, as well as posted to this ad, on December 28, 2023.
