Remodeling of a mixed-use development in Fort Wayne, Indiana. Completed plans call for the remodel of a medical facility.

Overview: The following describes the project Statement of Work (SOW) and provides an overview regarding the requirements for the specific projects identified. This project is located at the Veteran Affairs Northern Indiana Hospital Care System (VANIHCS-FTW) at 2121 Lake Avenue in Fort Wayne, Indiana 46805. This Project consists of remodeling of Building #3. Statement of Work: Building 3 houses the VANIHCS-FTW Executive Leadership Team. The proposed Remodel Building 3 project consists of the following activities. HAZARDOUS MATERIAL ABATEMENT: as indicated in the Construction Documents, specifically: Lead based paint. EXTERIOR MODIFICATIONS including: Demolition and replacement of entrance / stoops indicated in the Construction Documents. Demolition and reconstruction of main entrance steps and ramp including new sidewalks, curb ramps, etc. as indicated in the Construction Documents. Construction of three new entrance canopies, including foundations, columns, light fixture, roofing, and accessories as indicated in the Construction Documents. Hardscape Landscape PHASING PLAN: The following phasing is based on the priority as directed by the Fort Wayne VAMC. PHASE I - SOUTH [MAIN] ENTRANCE CANOPY, SIDEWALK and ACCESS RAMP: See Hazardous Material, Civil, Structural, Architectural, and Electrical Drawings for more information. PHASE II - WEST ENTRANCE CANOPY, DOOR 100A, FRAME and STOOP: See Civil, Structural, Architectural, and Electrical Drawings for more information. PHASE III - NORTH END / EAST FACING ENTRANCE CANOPY, DOOR 122A, FRAME and ACCESS SIDEWALKS; SOUTH END (near basement stairs) / EAST FACING DOOR 112A, FRAME and STOOP; NORTH FACING EXTERIOR ENTRANCE STOOP: See Civil, Structural, Architectural, and Electrical Drawings for more information. All work to be completed at the VANHICS - FTW, 2121 Lake Ave., Fort Wayne, IN 46805, VA Northern Indiana Health Care System Construction shall be performed in accordance with the drawings and specifications. The contractor shall complete this project within 168 days as specified in this SOW following issuance of the Notice to Proceed. Remodel Building 3, Project #610A4-21-206 PROJECT DESCRIPTION: The contractor shall provide all tools, equipment, labor, and supervision necessary to accomplish Nall work contained within the statement of work, specifications, and drawings. Contractor will provide all labor, equipment, materials and supervision necessary to completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for 610A4-21-206 Remodel Bldg. 3 as required by all drawings and specifications. Period of Performance is 168 calendar days after receipt ofNotice to Proceed. An organized pre-bid site visit is scheduled for: Date: 06 March 2024 Time: 13:00 Local Est Location: Ft. Wayne, IN VA Medical Center, Engineering Office Magnitude of Construction: Between $500,000 and $1,000,000 NAICS: 236220, Commercial and Institutional Building Construction Small Business Size Standard: $45 Million This solicitation is 100% set-aside for a Service Disabled Veteran Owned Small Business. See VAAR 852.219-74. NOTICE: The VA and/or a third party (support contractor) will require access to VA contractor records to assess contractor compliance with the limitations on subcontracting requirements included in VAAR 852.219-75. See Limitations on Subcontracting - Monitoring and Compliance (JUL 2018) located in the General Conditions. Bid Opening: Only e-Mailed Bids will be accepted. It is the responsibility of the offeror to ensure all emailed bids are received by the Contacting Specialist at the following email address: christopher.lee33@va.gov. Bidder's VetCert listing must appear at the time of bid opening (VAAR 819.7003(b)) AND at the time of award. Bidders should also be advised that the "fast-track" retrack" re-verification program ended September 30, 2012. Consequently, bidder appearing in VetCert at the time of bid opening AND at the time of award but only with the "re-verification" moniker will not be considered and will not be "fast-tracked" for re-verification. NOTES: (1) Failure to provide all documentation required by this solicitation may be grounds to deem your offer nonresponsive. REQUESTS FOR INFORMATION (RFI): To obtain clarifications and/or additional information concerning the contract requirements, specifications and/or drawings, submit a written RFI utilizing the RFI Form attached to this Solicitation, send via email to christopher.lee33@va.gov. The subject line of the email must read 'RFI - 36C25024B0009' RFIs must be specific identifying section, paragraph and page no. of the specifications or cite the drawing number and must be in question format. All RFIs submitted shall include the solicitation number and title, contractor name, city, state, telephone, email address, date submitted, and the RFI question(s). RFI responses will be posted to https://sam.gov/ as necessary in amendment format. Deadline for submission of RFIs for this solicitation is close of business 03/18/2024. No questions will be answered after this date unless determined to be in the best interest of the Government as deemed by the Contracting Officer. Telephone requests for information will not be accepted or returned. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
