This project will reconstruct roads in Districts 3 and 4; Vinal Street, Baden Street, Southview Drive, Airline Avenue, Brentford Court, Westbury Court, Melmad Court, Woodland Avenue, and Glenwood Road. Additionally, Vinal, Baden, Woodland, and Glenwood will all have new waterline installations. This project will reconstruct various roads throughout Districts 3 and 4 within the City of Toledo. This includes pavement removal and replacement, new curbs, drive approaches, and sidewalk. The project also includes new waterline installations on several of the reads scheduled for reconstruction. The quantities specified are estimated and informative only and not a binding offer of purchase on the part of the City. The City of Toledo reserves the right to either increase or decrease the contract quantities BIDDING INSTRUCTIONS Bids for this project can only be accepted electronically. All lines shall be quoted. All bids must have the required paperwork and bid guaranty attached with this e-bid in order to be deemed responsive. If you are e-bidding and are using a check as the bid guaranty, the check must be received by the Division of Purchases and Supplies no later than the due date and time to be deemed responsive. The bid guaranty may be a bid or a certified check or cashier's check on a solvent bank. The city of Toledo has a municipal income tax that is applicable to all contractor work performed in the city. This contractor activity is covered under Section 798.01 of the Toledo Municipal Code which requires tax registration when contractor work is performed. In order to become registered with the city of Toledo, Division of Taxation, you must complete the questionnaire on page 2 of this letter. The rate of Toledo municipal income tax is two and one-SAuI percent (2-1/%). As an employer, you are required to withhold municipal income tax from all compensation paid to your employees while working inside the city of Toledo. You must also pay on net profits attributable to Toledo projects. Completing this questionnaire will enable us to furnish the proper forms to you to effect compliance with the Toledo Income Tax Ordinance and the Contractor - Tax Registration Ordinance.
