(a) All pump testing will be done at the City of Ottawa Yard at 2799 Swansea Crescent or the Ottawa Fire training center (900 Industrial Rd) on a mutually agreeable date between the Contractor and the City. The Contractor shall be able to provide all equipment required to test on predetermined site approved by the City. The Contractor shall furnish all necessary supervision, labour, tools and supplies to provide pump testing on the City's fleet of Fire Vehicles. (b) The Contractor shall provide completed Service Test Results forms showing the analysis of the test on each Pump including the unit number and all pertinent information. (c) The Contractor shall not perform tests on any unit(s) without Maintenance Supervisor or Lead Licenced Technician authorization and a Work Order. (d) The Contractor shall contact the Maintenance Supervisor or Lead Licenced Technician when the testing of such a unit has been completed. (e) Upon completion of the test, a clear and accurate copy of the Work Order, consisting of all costs and a completed Service Test results form (see Appendix 1 Example Forms) shall be submitted to the City. This will include any and all recommended repairs in the "Remarks section" of the test results form. (f) All test gauges for pump testing must be calibrated and tested for accuracy and have documentation for each test gauge. The document must match the gauges by serial number. This document must be present before pump testing. (g) Testing shall be done as per National Fire Protection Association (NFPA) 1911. The Service Testing of Fire Pump Systems on Fire Apparatus Test Data Form (see Appendix 1 for details) shall be completed for each Pump and submitted at the end of each test to the Maintenance supervisor or Lead Licenced Technician. An actual blank Service Testing of Fire Pump Systems on Fire Apparatus Test Data Form is attached as the last page of this document. (h) Recommendations shall be recorded and properly documented on each inspection with full details of the problem. Any pump failures shall be reported immediately to the Maintenance Supervisor or Lead Licenced Technician on site. (i) If a pump fails within the first fifteen (15) minutes and the repairs are completed the same day, then a second (2nd) pump test will be performed on said pump he same day at no additional charge. If the pump fails after the first fifteen (15) minutes, then a second (2nd) test will be performed on said pump at the rate quoted under Price. (j) All pump(s) tested under this Agreement shall be inspected by the City prior to their acceptance. If a pump fails inspection after the first fifteen (15) minutes (see Section 2.1.9) the City may, at its sole discretion, choose to have the pump removed from service. (k) By means of an email to the Maintenance Supervisor or Lead Licenced Technician, the Contractor shall certify each pump was tested and certified under the NFPA 1911 standard. Commercial General Liability insurance issued on an occurrence basis for an amount of not less than $5,000,000 per occurrence for any negligent acts or omissions by the Contractor. Such insurance shall include, but is not limited to, bodily injury, death and property damage including loss of use: premises, property and operations liability; products and broad form completed operations liability; blanket contractual liability; cross liability; severability of interest clause; contingent employers liability; personal injury liability; owner's and contractor's protective coverage; non-owned automobile liability; broad form property damage; employees as additional Insured; occurrence property damage.
