NOTICE OF PROPOSAL AMENDMENT SOLICITATION NUMBER: 36C24124Q0184 AMENDMENT NUMBER: 0004 DATE: April 26, 2024 PROJECT NUMBER: 402-24-007 PROJECT TITLE: B200E Lobby Flooring LOCATION: Togus VA Medical Center, Augusta, ME 04330 REASON FOR AMENDMENT: Answer RFI Questions 1 thru 10 ___________________________________________________________________________ AMENDMENT: 0004 is the first amendment to solicitation, but automatically numbered 0004 due to previous amendments to the Sources Sought on SAM.gov. 1. Can you send over the most recent abatement testing report for the most accurate quote from the abatement subcontractors. VA Response There are no reports for abatement testing for building 200E. B200E was built in the 1990s and there should be no asbestos. We did have the slate tested just in case with reports showing no asbestos found.A 2. Floor Plan room E10, noticed green slate in there, would you like us to add this to the scope of work. VA Response Yes, all green slate flooring is to be removed. Please add room E10 to scope. 3. Dumpster location for demolition, where is the most appropriate area we may place a 15-yard + dumpster and not disturb the function of the VA. VA Response This will be coordinated with the COR.A 4. Phasing, foot traffic is heavy, with this, is there any areas the VA would like us to start/ section off first for demolition. VA Response Start by Emergency Department. 4B. From select demo: Can we have done the abatement and demo of tile done in one sequence or will this require mobilizing for parts of the SOW/ dividing it up. VA Response Per SOW Contractor shall also develop phasing and infection control plans that will allow for functional use of the lobby during construction operations. The VA believes no abatement will be required, only demolition. Per SOW When the contractor s work interferes with facility functions, such as when work produces excessive noise, odors, dust, utility service interruptions, or other interferences with normal operations that cannot be contained within the area of work, the contractor shall schedule said work at other than normal hours and as directed by the COR. This may be required during the demolition phase of this project. 5. Is poured terrazzo an option that fits the scope of work for this job. VA Response Poured terrazzo flooring will not be an acceptable option for this project. 6. Is there a preferred place we can place the furniture that is out so that it is out of the way during the demolition/ installation process. VA Response VA to move Lobby furniture. 6B. Who is responsible for the movement of existing furniture in the way of construction. VA Response VA to move Lobby Furniture. 7. Is black cove base by the stairs where the terrazzo ends and the set of stairs next to the elevator begins acceptable. VA Response This is acceptable. 8. Is the walk off matt staying. VA Response Walk off matt to remain in place. 9. How big would is the border intended to be/ Is the border going behind the front desk area. VA Response 4'x4' tile in the lobby and down to the pharmacy and emergency department with contrasting tile cut 1' along the border in hallways. If the 4' x 4' tile is too heavy and will require a forklift, we can accept the 2' x 2' tile at all locations.A 10. While reading the specs again, the Epoxy tile terrazzo, for clarification purposes, is this intended to be just tile or the concrete pour 4' x 4' terrazzo that can be poured into looking like tile. VA Response epoxy tile terrazzo only, addressed in # 5 as well. The tile we are looking for is a prefabricated tile. Terrazzio - Authentic Large Format Terrazzo Tile There should be no pouring needed. Precast Terrazzo Project Title: B200E Lobby Flooring Project Number: 402-24-007 Project Location: Togus VA Medical Center, 1 VA Center, Augusta ME 04330 Administered by: Alexandru Sava, Alexandru.Sava@va.gov The magnitude of construction is between $100,000 and $250,000. The NAICS code for this procurement is 238330 and the Small Business Size Standards is $19 million. Brief description: Removing existing lobby flooring on the ground floor of building 200E including corridors E70, E71, E72 and C70, approximately 4,650 sq. ft. of surface. The purpose of the project is to fully remove the existing failing tile, prepare the underflooring and install new terrazzo tiles. This acquisition is 100% set aside for Service-Disabled Veteran Owned Small Business (SDVOSB) concerns. Offerors must be both Visible and Verifiable by the Small Business Administration (SBA) in order to compete for this award. The Contractor shall be responsible for field verifying all conditions and quantities that may affect the project work prior to submitting quotes for this construction project. Participants are encouraged to attend and meet for a Site Visit at the VA Medical Center, Building 200E lobby, 1 VA Center, Augusta, ME 04330. A Site Visit will be held on Monday 04/15/2024 between 11:00AM and 1:00PM EST. POC for this action will be Alexandru Sava at Alexandru.Sava@va.gov. Request for Information of a technical nature must be provided in writing and shall be submitted by the prospective offeror only by email to Alexandru.Sava@va.gov. Email subject line must read: 402-24-007 B200E Lobby Flooring. Questions shall be received by Wednesday 04/24/2024 at 3:00PM EST. Period of Performance: work to be completed in 210 days from the issuance of the Notice to Proceed by the contracting officer in order to conduct operations from mobilization to demobilization for the demolition of the existing slate tile and installation of new flooring system. Offerors are invited to submit their responses to this RFQ by 05/06/2024 at 3:00PM EST. Responses are to be submitted only via email to Alexandru.Sava@va.gov. The Government intends to award a Firm Fixed Price contract to one offeror. The procurement will be a single award on a best value basis to the responsive, responsible contractor which will be providing best value to the Government. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
