The Navajo Area Indian Health Service (NAIHS), Kayenta Health Center, located at Highway 160, Mile Post 394.3, Kayenta Arizona is looking for a Contractor to Furnish and Install 9 Door Vision Panels in strict accordance with specifications required by Kayenta Health Center. Services must be completed IAW attached Statement of Work Project Title: Door Vision Panels Installation at Kayenta Health Center Notice ID#: IHS1482695 PRODUCTS NEEDED: Furnish and Installation of nine (9) Door Vision Panels on wooden doors Period of Performance: Award Date to 12/31/2024 Shipping Address: Kayenta Health Center Highway 160, Mile Post 394.3 Kayenta, Arizona 86033. In accordance with the Buy Indian Act, 25 U.S.C. 47, the Indian Health Service shall give preference at all times, as far as practicable, to Indian economic enterprises. If your firm is capable of providing the supplies and/or services described in this notice, complete the attached representation form and submit it along with the rest of the requested documents identified in this notice. EVALUATION CRITERIA: This procurement is being conducted under FAR Part 13 Simplified Acquisition Procedures. The Government reserves the right to issue a firm-fixed price single award or multiple awards to the contractor(s) whose offer represents the Lowest Price Technically Acceptable (LPTA); award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. Kayenta Health Center (KHC) is located south of Arizona State Highway 160 at milepost 364.3. The nearest town is a two hour drive. The Kayenta Service Unit serves a population of 20,000 people spread across a remote and sparsely populated area. Services are provided to 200 patients each day. KHC provides walk-in clinics and a 24 hour, 7 days per week emergency room. KHC requires professional services and expertise to perform the following work: a. Installation of nine door vision panels on wooden doors. 1. The contractor shall prepare a proposal, and upon award, shall provide an accurate timeline to complete project and following will perform the work to completion. 3.2. The project includes the following door vision window installation: a. Modification of door to accommodate vision window installation. b. Modifications need to maintain and not degrade integrity of door. c. All hardware need to be tested and certified (Ex. UL certified) d. Contractor to refinishing work will be necessary if any walls, ceilings and etc. is disturbed for work. e. Prepare existing door to place new vision panel f. Remove door to cut out box for vision panel. g. Contractor to furnish and install nine new door vison panel h. Test and certify vison panel secured and sealed Period of Performance 8.1. The period of performance shall commence immediately upon award
