Maricopa County Special Healthcare District dba Valleywise Health is seeking a qualified Construction Manager/General Contractor to serve as the Construction Manager at Risk ("CMAR") for Valleywise Community Health Center-Chandler-Refresh Project This Project Description outlines the scope for Valleywise Health for the refreshing and updating various areas at the Valleywise Community Health Center-Chandler. The Valleywise Community Health Center - Chandler (CHD) is located at 811 South Hamilton Street in Chandler, Arizona. The purpose of this work is to renovate the pharmacy, dental and registration areas to bring the clinic up to new clinic standards and provide better use of the space. The following is a summary of the current areas that we are looking at refreshing and redesigning: Dental Clinic - 826 SF Pharmacy - 274 SF Lobby/Restroom - 1,731 SF Staff Lounge - 275 SF Construction Area The attached set of drawings highlight the scope of area for construction for Chandler Clinic Refresh. The proposed plan involves construction to be complete in three phases. This allows the services to be functional while construction is completed in each phase and the service relocated to that area. The design will happen in a single phase. Phase I - Move registration to front lobby and renovate this area for Dental Clinic Phase II - Move Dental clinic to area constructed in Phase I and renovate current Dental for Pharmacy Phase III - Move Pharmacy to area constructed in Phase II and renovate front lobby for Registration and clinic office spaces. Dental Area Requirements: Dental Clinic Complete work as designed from input from the Dental manager. Area will require a design using input from the site and Dental managers. Renovate old registration area to accommodate: o 5 patient care dental chairs / bays o Pass through Xray units between 2 bays; decreases the need for an XRAY Room and saves space like McDowell Dental o Standalone Panoramic XRAY niche; like McDowell Dental shown Total construction cost is estimated to be between $ 2M-$3M.
