Provide plans to furnish and place an aluminum box culvert with 34-8 span, remove and dispose existing pipes, provide roadway materials, stream restoration, habitat improvements, scour hole repair, dewatering, erosion and sediment control, traffic control, site work and restoration in accordance with contract documents and permits. PA Prevailing Wage Rates Apply. The successful Bidder must comply with the Public Works Employment Verification Act. The Contract Documents contain information and a verification form regarding the Act. Each Bid must be accompanied by a Certified or Bank Check or Bid Bond payable to the order of the Owner in the amount of ten (10%) percent of the total amount of the Bid as security for proper execution of the Agreement and submitting other documents as required by the Bidding Documents. The successful Bidder will be required to execute the Agreement and to furnish a performance bond and a payment bond, each of said bonds to be in the amount of one hundred percent (100%) of the Contract Price, all in the respective forms provided in the Bidding Documents. The successful Bidder will also be required to furnish evidence of insurance coverage as required by the Bidding Documents. The Municipality reserves the right to reject any or all proposals. The Project for which the Work under the Contract Documents may be the whole or only part is generally described as follows: Provide all plans, labor, equipment and materials required to demolish, remove, & dispose of an existing stream crossing consisting of two (2) steel culverts approximately 11-feet diameter by 40-feet long, construction of an aluminum box culvert with 34'-8" span, 8'-9" nominal rise, and 31'-6" length, stone road surface, stream restoration, habitat improvements, filling in existing scour hole, handling stream flow diversion/pumping, erosion and sedimentation control, traffic control, all site work and excavation required to complete the project and site restoration, all in accordance with the Contract Documents.
