Page 2 of 2 Project #: 630A4-24-411 Project Title: Renovate D2 Swing Space (St. Albans) Scope of Work: Provide all necessary construction services for the above project at the New York Harbor Healthcare Center St. Albans Community Living Center St. Albans, NY. Schedule: 60 calendar days Specific Requirements: This project includes a general cosmetic renovation/upgrade of the entire second floor (D2 Ward) of Building 86 to accommodate approximately 20 beds in the ward. Hallway/Main Corridor: Purchase and install automatic door equipment for the ward s front doors to make them automatic. Relocate, if necessary, existing electrical fixtures, LED lights, outlets, switches, cover plates, and possible wiring and conduits Repair of exhaust line (in conjunction with ongoing C3/D3 project) Seal/caulk existing VCT/terrazzo floor tiles with gray sealant at cracked locations only Installation of VCT floor tiles (whenever the tiles are missing or broken) to be spot-patched. Secure and tighten a loose wall-mounted handrail. Cut, remove, and replace only damaged portions of existing baseboard/molding. Purchase and install new full-height corner guards that are missing or damaged (9 corner guards) Plaster/grout, spackle, and caulk all holes and defects throughout the hallway, as needed. Purchase and install five (5) window-mounted air conditioners and associated louvers. Install new dedicated power feed for new AC units. Demolition and removal of existing rail support for ten (10) window curtains Purchase and installation of blinds and rail support for ten (10) window curtains Replace one damaged radiator cover. Clean all areas once all work is complete. Adjust misc. Existing doors and hardware as needed to close and latch properly. Purchase and installation of two (2) new 46 x 80 HM doors with new hardware and hinges Restore each headwall in each room. Installation of one (1) sink for all patient rooms. Replace all radiator traps and controls. Contractor must follow all VA infection control, safety, and security requirements. Relocate sprinkler heads as needed. Install window ACs in each room. BTU is determined by the square footage of each room. Reuse AC split units in room D205 to feed the main corridor. Four ceiling cassettes are to be spread equally throughout the corridor. Contractor responsible for connecting televisions to the existing VA cable network. All showers to have scald protection installed. Requirements: A. Construction contractors and A/E design firms selected by award to perform work at VISN 2 VA facilities must utilize the VISN 2 Autodesk Build Construction management platform, also called Autodesk Construction Cloud (ACC). Access to the platform and training portal will be provided by VISN 2 through the project Contracting Officer Representative (COR) at no cost for the duration of the project. Autodesk Build is the management and collaborative environment that VISN 2 uses for all its Non-Recurring Maintenance (NRM), Minor Program projects, Feasibility Studies, Commissioning, and Retro-Commissioning contracts. The contractor has no user, license, or subscription fee for using this cloud-based platform. B. Contractor shall accomplish and complete the following tasks upon award of contract: 1. Request access to the ACC platform and Productivity NOW eLearning Site to project VA COR NLT 14 days from contract award. 2. Only staff utilizing the platform will be granted access examples: Project manager, sub-contractors POC, Site manager, Safety officer, etc. 3. The access request shall be in writing and include a staff list. The list shall include the following information: A. Full Name A. Company Name A. Email Address A. Role/Position on the project (i.e., Project manager, site super) 4. Upon granting access, staff shall complete the required ACC training through the Productivity NOW platform within 12 Calendar days. 5. Upon successful completion of training, the contractor shall submit certificates of completion to COR NLT 2 business days. C. Access to Productivity NOW eLearning will expire after 12 Calendar days after online access is granted. The contractor shall complete ACC training within that period. D. Additional staff and subcontractors can be added anytime with a request submitted to the project COR as described above. E. The contractor shall only use the ACC platform for contract-required submissions and official project correspondence to be acknowledged, reviewed, and action by the responsible party. No additional time shall be added to the contract, nor will an increase in contract amount be provided for the contractor s failure to utilize the ACC platform as the project s official communication and collaboration system. Any correspondence from the platform shall not be the governing authority's contradicting direction and will be at the contractor s expense. Official project correspondence and collaboration include but are not limited to RFIs, submittals, schedules, shutdown requests, actionable tasks, reports, testing, safety inspections, photos, and site surveys. F. Contractor must inform the COR of any staff changes NLT 2 business days from the change. Staff changes pertain to staff who was granted access to the ACC platform. Contractor shall request termination of access, replacement of personnel and/or any other action that might impact the contractor ability to maintain required utilization of the platform. G. Contractor submission requirements cannot be accepted for work to begin without meeting all ACC Platform requirements outlined herein, including completing required ACC platform training. The Method of Contractor Selection has not been determined at this Time.
