Milling of 5,330 SY +/- of Existing Road Surface Installation of 5,330 SY +/- of Hot Mix Asphalt Surface Course, 2" Thick Installation of 350 LF Concrete Curb Each Proposal Bid Must Be Accompanied by: a Proposal (Bid) Bond or Certified Check for Not Less Than Ten Percent (10%) of the Amount of the Bid, of Which, the Amount of the Bond or Certified Check Need Not Exceed $20,000, Made Payable to the "City of Woodbury"; a Non-collusion Affidavit; and a Consent of Surety as to a Maintenance Bond as Per N.j.s.a. 40a:11-16.3b. The Successful Bidder Will Be Required to Furnish, Within Ten (10) Days After the Award, a Surety Corporation Performance Bond or Letter of Credit Equal to One Hundred Percent (100%) of the Contract Price. All Blank Spaces in the Proposal, Except as Otherwise Noted, Must Be Filled in; and No Change Shall Be Made in the Phraseology of the Proposals or of the Item Mentioned. All Information That May Be Requested of the Bidder in These Specifications Shall Be Included With This Proposal. All Erasures, Interpolations, or Other Physical Changes on the Bid Form Shall Be Initialed or Signed by the Bidder.
