Lee County Port Authority requests competitive quotations from suppliers to provide the items and/or perform the services that are the subject of this Request for E-Quote in accordance with the specified Attributes. The Lee County Port Authority (Authority) seeks a qualified quoter to provide the items/services identified herein in full accordance with the requirements published in this Request for Quote and any agreement entered into with the Authority. Vendors contracting in a corporate capacity must be registered with the Florida Department of State, Division of Corporations, as a Florida corporation or other Florida recognized and approved legal business entity in good standing and authorized to conduct business in the state of Florida. Wherever the term "vendor" is used herein, it should be understood to mean the 'successful Quoter(s)'. SCOPE OF WORK / MINIMUM SPECIFICATIONS SCOPE OF SERVICES Provider will be required to provide inspection and testing for fire alarm and detection equipment located on Southwest Florida International Airport property in accordance with the terms, conditions, and scope of work contained in the Request for Quotes and this Scope of Services, Provider must supply all material, labor, parts, supervision, tools, and transportation necessary to perform the services in accordance with this Scope of Services. Provider must provide prompt and convenient service in accordance with this Scope of Services. The Authority reserves the right to obtain products or services from other sources in the event the Provider is unable to perform on a timely basis. The following locations require fire alarm testing and inspection services. The fire protection equipment corresponding with each location is identified below. 1. Main Terminal and Rent-A-Car Buildings Address: 11000 Terminal Access Road, Fort Myers, FL 33913 Description of System: Edwards Model Number: EST-3 Alarm Initiating Devices & Alarm Indicating Appliances Main Station, Photo Detectors, Duct Detectors, Heat Detectors, Pull Stations, Ansul Monitors, Strobes, Horn Strobes, Audio Visuals. 2. Chill Chiller Building Address: 11000 Terminal Access Road, Fort Myers, FL 33913 Description of System: EST Model Number: 3 Alarm Initiating Devices & Alarm Indicating Appliances Main Station, Photo Detectors, Duct Detectors, Heat Detectors, Pull Stations, Strobes, Horn Strobes, Audio Visuals. 3. Authority reserves the right to add or delete Alarm Systems and Alarm locations during the term of this Agreement. For any additions, the Authority will first request Provider provide a quote for the added system or location. Quote pricing must be substantially similar to the pricing for comparable systems or locations. If the Authority accepts the quote, the Provider will provide service to the added system or location for the remaining term of the Agreement. ANNUAL TEST AND CERTIFICATION INSPECTION Provider must conduct an annual test and certification inspection of the alarm systems at each of the listed locations that meets the following requirements: The annual test will be a 100% functional test of all devices and must be performed in accordance with National Fire Protection Association standards. Annual inspections must be performed in accordance with all applicable National Fire Protection Association (NFPA) standards. Provider must ensure all panels are tagged to comply with Fire Marshall Standards. Upon test completion, the Provider must provide a certification on standard NFPA forms to document the results of each test and certify the state of the system. The certification must be filled out completely for every location. An NFPA 72 compliant Inspection report must be provided for each annual inspection conducted and any time an inspection is requested or required by the Authority in order to ensure proper operation of equipment. WORKING HOURS & RESPONSE TIME Services will be performed between the hours of 8:00 P.M. - 4:00 A.M. Monday through Friday. Authority will not pay for travel time. Time will start when the Provider arrives on site and ends when they leave. Time will be billed in thirty-minute increments. Provider shall coordinate a schedule with the Authority, subject to Authority approval, that indicates the exact time the inspection shall occur at each location. REGULATIONS Provider must comply with all applicable federal, state and local laws, ordinances, rules and regulations pertaining to the performance of the work specified herein. Provider must obtain and keep current all permits, licenses, and certificates, or any approvals of plans or specifications as may be required by federal, state, and local laws, ordinances, rules, and regulations, which are necessary for the proper execution of the work specified herein. Upon request of the Authority, the Provider must provide copies of its license(s) registrations and/or permits. Permits must be displayed on the job site during the performance of the work. Provider must immediately report any spillage or dumping of hazardous materials caused or made by it or its subcontractor(s) on Authority property to the Authority. Provider must be responsible for all cleanup and any related costs incurred for such incidents. PROVIDER'S PERSONNEL Provider's personnel must be trained to work on specified alarm systems, including software and programming. They must provide all services professionally and have backup technical support available if necessary. A qualified, English-speaking supervisor must be present to oversee work, and all personnel must be trained to identify signs and follow instructions. The provider must maintain a drug-free workplace and promptly remove unsatisfactory employees. All found articles must be returned to the Authority. Motor vehicle operators must have a valid license and display the provider's logo on vehicles. Staff must wear clean, safe clothing and footwear, and follow the requirements governed by OSHA. BASIS OF AWARD The award will be made to the responsive and responsible Quoter having the lowest total quote. The lowest Quoter is the Quoter submitting the lowest quote for goods and/or services of a quality which meets or exceeds the minimum specifications/scope of work set forth in the E-Quote documents. To be eligible for consideration, all quote lines must be completed. A n/a, blank, or zero will be deemed to mean "no charge" to the Authority To be responsive, a Quoter must submit an E-quote that conforms in all material respects to the requirements set forth in the Request for E-Quote. To be responsible, a Quoter must have the capability in all respects to perform fully and the tenacity, perseverance, experience, reliability, integrity, capacity, facilities, equipment and credit which will ensure good faith performance. The Authority reserves the right to make such an investigation as it deems needed to ensure the ability of any Quoter to furnish requested services. Information the Authority deems necessary to make this determination must be provided by the Quoter. Such information may include, but will not be limited to, current financial statements, verification of availability of equipment and personnel, and past performance records.
