Please send questions about the content of this RFP to Sonoma Water at Submissions@scwa.ca.gov. Please reference TW 23/24-033 in the subject line of the email. If Sonoma Water considers interpretations or clarifications necessary, Sonoma Water will provide a written supplement to this RFP. Proposals shall include the following: a. A table of contents and page numbers. b. Legal name of company, how organized (non-profit, LLC, etc.), and where company is incorporated. Not required for individuals. c. Screenshot of registration with the Department of Industrial Relations showing effective date and expiration date. (See Section 4 above.) d. A statement of similar work previously performed, including at least three references with name of organization, contact person, and telephone number. e. A statement of qualifications and a list of personnel to be assigned to the work, including a resume for each, listing education, experience, and expertise in this type of work. f. A list of persons or firms to whom any phase of the work may be subcontracted, including a statement of their qualifications and experience. g. A description of the project including the project name and location, project size, map(s), project goals, and the project components and activities. Describe how the project will provide flood control, water quality, habitat, or species-specific benefits. h. A description of how the project meets the WPP requirements (see Section 3.7, above) and which, if any, desirable features (see Section 3.8, above) are included. i. If the project includes native plant revegetation, a description of the plant types (such as trees or shrubs), layout, approximate quantities, and a planting irrigation plan. j. A description of the required permits or approvals proposer will acquire and be responsible for complying with to perform the work, such as environmental permits, right-of-way access, and landowner permission. Attach supporting documents, if applicable. k. A five-year maintenance, monitoring, and reporting plan. Describe how the project will be maintained and monitored to ensure and quantify effectiveness.
