Work Items Include: Milling of Existing Pavement, Tack Coat, Resurfacing With Hma and Appurtenant Work. MassDOT prequalification of contractors with the class of work as, Pavement Surfacing, for the project with an estimated value of $1,473,172.00 will be required Only contractors that are prequalified to perform the class of work for a project value noted above are eligible to bid on this project. MassDOT issues the list of contractors meeting this requirement up to the day of the bid opening. Only contractors listed on the MassDOT prequalification list or waiver list will be allowed to submit Official Bid Forms. Any bidders that are NOT on the MassDOT Prequalification or Waiver lists, issued the day of the bid opening, will be consider in eligible to submit a bid on this project. A 5% bid bond is required and must be submitted with the bids. The contractor selected to perform work will be required to provide 100% payment and performance bonds. Firms must be able to supply insurance certificates indicating coverage as noted in the bid documents and the contractor will be required to sign an indemnity agreement prior to award of work under this bid. All labor performing work on this contract must be paid in accordance with the Commonwealth of Massachusetts approved wage scales. Certified payrolls must be provided to the town for all work performed. Firms submitting prices must be equal opportunity and affirmative action employers.
